AIm: Postoperative cerebrospinal fluid (CSF) leak following endoscopic endonasal transsphenoidal surgery (EETS) is associated with increased morbidity and mortality. This prospective study is the first evaluation of using autologous fibrin sealant for preventing postoperative CSF leak and related complications. mAterIAl and methOds: 200 endoscopic endonasal transsphenoidal approaches were included in the study and reviewed retrospectively from September 2010 to June 2012. A total of 55 patients who have large skull base and diafragma sella defects, connected with basal cisterns or ventricles, were chosen for the study. The patients were operated via extended or classical endoscopic endonasal transsphenoidal approach. The skull base has been repaired using AFS combined with multilayer reconstruction in all cases. The incidence of CSF leak as a complication of EETS was analyzed. results:The ages of the patients ranged from 20 to 83 years (mean 49.3 years). There were 25 (46%) male patients and 30 (54%) females. All patients had tumors with suprasellar or parasellar extension. Postoperative CSF leak was determined in 2 patients (3.6%). There were no complications and allergic reactions associated with the use of AFS.COnClusIOn: Using of AFS combined with multilayer reconstruction technique is a safe and effective method to prevent CSF leak in large defects following EETS.

Aim: We present our experience with partial nephrectomy using radiofrequency energy in patients with kidney cancer. Methods: From July 2002 to February 2007, 15 radiofrequency ablation assisted partial nephrectomies were performed. Five patients had one kidney only. The average tumour size was 34 (range 22–55) mm, and the mean age was 56 (range 45–69) years. The renal pedicle was not clamped. A monopolar cool-tip radiofrequency device under intra-operative ultrasound guidance was used. Our procedure involved inserting a radiofrequency probe around the tumour. The average ablation time was 6–12 min. Afterwards the tumour was removed and analyzed histopathologically. Results: The average follow-up period was 15 (range 1–42) months. Eleven procedures were performed without any complications. In 4 cases, delayed urine leakage was observed. Histopathological examination revealed 13 clear cell renal cell carcinomas and 2 oncocytomas. In all cases the coagulated surgical margins were negative. Currently, the surviving patients have a well-preserved renal function, and there was no tumour recurrence. Conclusions: Radiofrequency ablation assisted partial nephrectomy seems to be an effective method of nephron-sparing surgery. The major advantages of this procedure include easy tumour extraction and no signs of bleeding.

renal cell carcinoma is an urgent problem in urologic oncology practice. The wide use of radiodiagnostic methods has increased the detection rate of small localized tumors, leading to an increase in the number of patients, for whom organ-sparing surgery is indicated. Kidney resection is the most common, effective, and relatively safe treatment option in this contingent of patients. At the same time, kidney resection is related to the need for hemostasis during operations on the actively vascularized organ. Thus, adequate hemostasis is the most important surgical stage during kidney resection. hemostatic procedures are diverse and include different mechanical, physical, and chemical exposures of the vascularized parenchyma of the organ. The literature covering different hemostatic techniques during kidney resection are reviewed. The results of large-scale studies assessing different hemostatic procedures are given.
