Autologous Bone Grafting in Trauma and Orthopaedic Surgery: An Evidence-Based Narrative Review

Abstract: Autologous bone grafting is common in trauma and orthopaedic surgery. Both the Reamer Irrigator Aspirator (RIA) and Iliac Crest Bone Graft (ICBG) aim to obtain autologous bone graft. Although the process of harvesting a bone graft is considered simple, complications may occur. This study examined morbidity and pain at the donor site, blood loss, and iatrogenic fractures, comparing RIA and ICBG. The source of the autologous bone graft, the alternative graft sites, and the storage modalities of the harvested bon… Show more

“…The "point-to-surface contact" between the titanium mesh cage and the endplate results in a very high-pressure intensity on the contact surface, making it easy for a stress-shielding effect to occur [32][33] . In contrast, the n-HA/PA66 strut ends have a wider surface to form "face-to-face contact" with the endplate, which can effectively distribute the load on the contact surface, avoiding cutting effects [21] . In addition, The grooves at both ends of the n-HA/PA66 strut are designed to increase the friction between the cage and the endplates can effectively avoid the sinking and displacement of the strut.…”

“…This type of graft cannot induce adverse immune reactions because it is autologous, has osteogenic, osteoinductive, and osteoconductive capabilities and stimulates osteoproliferation with rapid healing. However, the need for an additional invasive operation, increased morbidity, pain, and the difficulty of the harvesting areas, reduce the rates of use of autologous bone grafting [ 6 ]. Furthermore, growth factors are released only after the graft has been completely resorbed [ 7 , 8 ].…”

“…Knowledge of the mechanical properties of cancellous bone is beneficial not only for understanding degenerative bone diseases such as osteoporosis and osteoarthritis [ 1 3 ], but also for the development of bone grafting techniques and substitute materials, as well as their clinical application in trauma and orthopaedic surgery [ 3 5 ].…”
