Autoimmune pancreatitis: radiologic findings in 20 patients

Abstract: In this series, common radiologic findings of autoimmune pancreatitis were (a) diffuse pancreas enlargement, (b) multifocal narrowing of the main pancreatic duct, (c) narrowing of peripancreatic veins, and (d) tapered narrowing of the distal common bile duct with frequent contrast enhancement. These findings were usually reversible with steroid therapy.

“…In our experience of 112 AIP patients, focal or segmental swelling was observed in 23.2%, and other authors have reported 9 -30% frequencies [45][46][47]. In addition, AIP with multifocal lesions has been reported [51][52][53][54] and was found in 7.1% of our cases (Fig.…”

“…To date, we have diagnosed 112 AIP patients and observed diffuse pancreatic swelling in 53.6% and a capsule-like rim in 29.5%. Other studies with large cohorts showed diffuse pancreatic swelling in 22 -95% of patients and a capsule-like rim in 12 -90% [45][46][47].…”

“…This non-invasive approach is useful not only for evaluating the location, distribution, and degree of the biliary strictures by MR cholangiopancreatography, but also for demonstrating other abnormalities such as bile duct wall thickening and unusual enhancement patterns using pre-contrast and dynamic contrast-enhanced MR images. In patients with concomitant AIP, the pancreas shows imaging abnormalities highly specific for the diagnosis (e.g., diffuse enlargement, capsule-like rim around the pancreas, and irregular narrowing of the main pancreatic duct) [39,40]; therefore, the diagnostic approach needs to primarily target the pancreatic lesion. CT and MR imaging also sometimes depict IgG4-RD in other intra-abdominal organs unexpectedly.…”

“…4,5 In an appropriate clinical setting, a constellation of imaging features is very helpful in the diagnosis of AIP. The abnormal findings are reversible with steroid therapy and thus imaging can be used as an evaluation of treatment response.…”
