Autoimmune inner ear disease: A systematic review of management

Abstract: Objectives: The study systematically reviewed the existing literature on the management of autoimmune inner ear disease (AIED). Study Design: Systematic review. Methods: We performed a literature search of Embase, NCBI, Cochrane, and Web of Science databases from April 1990 to April 2020. Inclusion criteria included studies that were retrospective or prospective in nature evaluating the treatment of AIED with audiometric data measuring hearing outcomes during treatment. Hearing improvement was the primary stud… Show more

“…Corticosteroids remain the primary medical therapy in autoimmune SNHL, with activation of glucocorticoid receptors within the cochlear hair cells causing downregulation of local cytokines and reducing autoantibody production and inflammation [ 20 ]. This attenuates cochleitis and hair cell death, with potential recovery of function.…”

“…Despite an increase in clinical awareness and in the knowledge of the immune mechanisms involved, AIED is still a challenging diagnosis as it is necessary to conjugate the presence of SNHL with data related to the clinical presentation, audiological testing and response to immunomodulatory drugs [ 7 ].…”

“…There is a common belief that AIED, specifically when associated with a SAID, is a progressive disease that involves both ears and many studies included only cases with bilateral hearing loss [ 8 ]. However, given that the clinical presentation of AIED can be sudden and unilateral [ 8 ], sometimes taking years to become bilateral [ 7 ], the authors chose to include patients with unilateral and bilateral hearing loss.…”

“…More recently, various biologics both systemically and intratympanically have been tested. There is little consensus as to the most effective treatment 16 . In some cases, however, the progressive nature of the AIED results in the need for hearing aids and/or cochlear implantation due to failure of medical therapy 12 …”
