Autoimmune diabetes induced by PD-1 inhibitor—retrospective analysis and pathogenesis: a case report and literature review

Abstract: Anti-PD-1 antibody treatment is approved in advanced melanoma and provides median overall survival over 24 months. The main treatment-related side effects are immune-related adverse events, which include rash, pruritus, vitiligo, thyroiditis, diarrhoea, hepatitis and pneumonitis. We report a case of autoimmune diabetes related to nivolumab treatment. A 73-year-old man was treated in second line with nivolumab at 3 mg/kg every two weeks for metastatic melanoma. At 6 weeks of treatment, he displayed diabetic ket… Show more

“…In the subset for whom autoantibodies were present at CPI‐DM diagnosis, it is unknown if they were present preceding treatment, potentially allowing for risk prediction, or whether they have developed during the course of treatment. Pretreatment serum was available for a limited number of cases, and these have had mixed results . It is also unknown if the presence or absence of these autoantibodies represents different forms of CPI‐DM.…”

“…Gauci et al reviewed data from 24 reported cases of DM after the use of ICIs. Although the presence of serum islet β‐cell‐related autoantibodies at the onset of diabetes supports the autoimmune nature of type 1 diabetes, only 11 of 22 patients tested (50%) had positive anti‐glutamic acid decarboxylase 65.…”

“…There is a lack of information regarding the haplotype testing in the cases published, but a few patients were found to have haplotype DRB1*04 as reported in this case 2 12 20. This haplotype has been suggested to contribute to the development of T1DM after anti-PD-1 therapy 7. Therefore, in the future, the search for specific haplotype can possibly help to identify patients at risk of DKA.…”
