Abstract:Anti-NMDA-R encephalitis is a relatively frequent form of autoimmune encephalitis. Initial clinical features of anti-NMDA-R encephalitis resemble those of schizophrenia exacerbation which resulted in hospitalization of patients to mental care facilities. Taking into account high lethality and potential curability, detection of this condition is an important clinical problem. The authors describe a case report of encephalitis with NMDA-R antibodies. This report is the first in the domestic literature. The diffi… Show more

Encephalitis with antibodies to NMDA receptors is a form of autoimmune encephalitis that most often debuts as an acute psychosis. Considering its curability and reversibility, encephalitis with antibodies to NMDA receptors must be included into differential diagnostic list for all the patients suffering first psychotic episode. However, to diagnose it, it is necessary to follow a step-by-step algorithm considering both clinical and instrumental and laboratory data. Confirming anti-NMDA-receptor encephalitis based solely on increased blood antibodies level often leads to false-positive diagnostics and errors in treatment of such patients. In this article, we discuss diagnostic features of encephalitis with antibodies to NMDA receptors,as well as other forms of autoimmune encephalitis that can initially manifest as acute psychoses.

The review focuses on autoimmune encephalitis with antibodies to NMDA receptors, previously unknown autoimmune disease of the central nervous system, described in 2007. Anti-NMDAR encephalitis develops in adults and children, due to the tumor process and without it, it is manifested by characteristic syndrome of neurological and mental disorders. The authors separate anti-NMDAR encephalitis, which develops after the resolution of encephalitis caused by the herpes simplex virus-1. They describe the clinical manifestations of anti-NMDAR encephalitis in children and the effectiveness of early immunotherapy. Perspectively they discuss the pathogenesis of the disease, as well as the connection of herpes simplex virus infection and the development of anti-NMDAR encephalitis.

After conducting a thorough analysis of clinical, neurophysiological, neuroimmunological, neurobiological, and neuropathological research findings, the authors of the review question on the validity of the anti-NMDA receptor encephalitis concept. The review highlights the significance of studying sanogenetic mechanisms in medicine and warns against hasty interpretations of neurobiological data without clinical knowledge.
