BackgroundMicrobiology reference laboratories are critical in the development of high-quality clinical and public health services. In Belgium, the reference laboratories performed their activities on a voluntary basis and lacked a legal status.MethodsPathogens or groups of pathogens necessitating a national reference center (NRC) were prioritized based on diagnostic and epidemiologic relevance. Terms of reference for each of these pathogens were developed.ResultsRecently, 40 NRCs for different pathogens or groups of pathogens have been installed in Belgium to fulfill the following core functions: offering reference diagnostics, collecting reference materials, sharing information and scientific advice, participating in national and international networks, collaborating with research workgroups, and contributing to surveillance activities.ConclusionsThese NRCs are important focal points of the national and international network in public health microbiology.

ZusammenfassungDas Robert Koch-Institut (RKI) bietet den Gesundheitsämtern in Deutschland bei Ausbrüchen von Infektionserkrankungen Beratung und praktische Unterstützung vor Ort an. Die Feldeinsätze werden von speziell geschultem Personal durchgeführt. Auch im Rahmen der COVID-19-Pandemie leistet das RKI regelmäßig diese Form der Amtshilfe in unterschiedlichen Settings. Dabei handelt es sich beispielsweise um Ausbrüche in Wohngebäuden, Arztpraxen, Alten- und Pflegeheimen, Kliniken, Erstaufnahmeeinrichtungen für Asylsuchende, aber auch in einem Nachtclub oder auf einem Kreuzfahrtschiff.Der vorliegende Beitrag berichtet exemplarisch von Feldeinsätzen, die im Zeitraum Februar bis Oktober 2020 im Rahmen der COVID-19-Pandemie stattgefunden haben. Die daraus gewonnenen Erkenntnisse tragen dazu bei, das Wissen zu SARS-CoV‑2 zu erweitern, z. B. zur Übertragung und Ausbreitung des Erregers, RKI-Empfehlungen zu formulieren oder zu untermauern und das Management komplexer Situationen zu unterstützen. Die Praxisbeispiele zeigen, wie vielfältig die RKI-Teams nicht nur vor Ort unterstützen, sondern auch die epidemiologische Evidenzbasis bereichern.Im September 2020 wurde im RKI die „Kontaktstelle für den öffentlichen Gesundheitsdienst (ÖGD)“ eingerichtet, die unter anderem die Amtshilfe durch Feldeinsätze koordiniert und erweiterte Beratungsmöglichkeiten bietet. Damit der ÖGD langfristig noch besser auf Ausbrüche von Infektionserregern reagieren kann, soll das interdisziplinäre Trainingsangebot intensiviert werden.

With pandemic influenza in 2009/2010 and an EHEC outbreak in 2011, the Federal Republic of Germany experienced two extensive outbreaks in the course of only 3 years. Although both infectiological crises were comparatively successfully coped with, certain aspects have been critically examined. One point of criticism has been the presumption that federal structures may not be well suited for the management of a nationwide outbreak. This has been linked to the request for a central authority with responsibility. In fact, centralized as well as federal structures have advantages and disadvantages during infectiological crises. However, the "first response," i.e., immediate action against the spreading of infectious diseases, has to take place locally anyway. Regional differences, even in the context of a nationwide outbreak, might well demand regional action. After all, the federal structure of the Republic of Germany is deliberately firmly rooted in the German constitution, and there are no indications that this may change in the near future. Suitable concepts and structures should be used so as to benefit from the advantages and avoid the disadvantages of a federal state. The current structures are described, and improvements that may be necessary are discussed. The existing structures are shown to be entirely appropriate in allowing necessary decisions to be made and a fast transmission of information even in a federal state. Occasional shortcomings are seen as mainly due to the inadequate implementation of already existing regulations and partly to the ambition of a few spotlight seekers rather than to actual inadequacies of existing federal structures.
