Augmentation of Circulating Follicular Helper T Cells and Their Impact on Autoreactive B Cells in Myasthenia Gravis

Abstract: Myasthenia gravis (MG) is a chronic humoral immunity-mediated autoimmune disorder of the neuromuscular junction characterized by muscle weakness. Follicular helper T (Tfh) cells may be the key Th cell subset that promotes MG development, as their major function is helping B cell activation and Ab production. Aberrance of thymus-derived Tfh cells might be implicated in autoimmune diseases including MG; just how circulating Tfh cells, especially those from patients with a normal thymus, contribute to MG pathogen… Show more

“…Recently, the role of Tfh cells in MG development was reported (35,42,43). To assess whether NFAT1 deficiency affects Tfh cell development, we measured the transcription factor Bcl-6, which is known as a major Tfh cell marker (44).…”

“…Tfr can suppress the production of IL‐21 and IL‐4 by Tfh cells and inhibit class switching and antibody production by B cells. In MG, the increase in Tfh cells is likely due to a dysregulation in Tfr cells because patients with MG exhibit lower ratios of Tfr/Tfh cells and frequency of Tfr cells . Going forward, a better understanding of the dynamics among Tfh, Tfr, and B cells in immune tolerance and disease pathogenesis is essential.…”

“…Previous data indicated that IL-21 counteracted the suppressive activities of CD4 1 CD25 1 T cells markedly on CD4 1 CD25 -T cells without affecting the percentage of FoxP3 1 cells or survival of T reg [16]. Conversely, increased serum levels of IL-21 were reported in AChR 1 MG compared to healthy individuals [21,22]. Here, we confirmed that IL-21 could increase the proliferation of T resp cells in T resp /T reg coculture in both AChR 1 MG patients and HC.…”
