Augmentation Mammaplasty by Reverse Abdominoplasty (AMBRA)

Abstract: Augmentation mammaplasty by reverse abdominoplasty is a versatile procedure that in the carefully selected patient can successfully address two aesthetic concerns simultaneously, providing durable autologous tissue that can obviate or enhance the outcome provided by prosthetic implants and rejuvenating the abdomen. It also shows promise as a significant adjunct to the techniques available to the breast reconstructive surgeon.

“…3 In recent years, this technique has been used on patients with massive weight loss and could be combined with circumferential abdominoplasty to use the infraumbilical laxity 9 and also for breast augmentation. 10 The main advantages were excellent alternative in complex cases, avoidance of microsurgical technique for free tissue transfer, simplicity of the technique to execute, and an acceptable aesthetic outcome. It could be associated with other techniques such as autologous fat grafting, and it could contribute with significant bulk.…”

“…The extended superomedial technique is indicated whenever autoaugmentation is required using vascularized breast tissue to fill defects or provide additional volume in certain locations within the breast mound. Variousautoaugmentation techniques have been proposed in the literature, mainly in relation to management of the massive weight loss breast [4][5][6][7][8][9][10][11]. Hamdi et al described using excess lateral tissue based on the lateral intercostal artery perforators to autoaugment volume depleted breasts [6].…”

“…The use of local flaps and autoaugmentation techniques has subsequently become popular option to rotate tissue into areas of volume void. These can be taken either from within the breast mound or locally [4][5][6][7][8]. The superomedial pedicle is ideal for an extension type pedicle since the removal of tissue in the lower pole is often desired to allow glandular plication and breast shaping.…”

“…This excess distal Plastic and Reconstructive Surgery • September 2010 reverse abdominoplasty tissue could be deepithelialized and used as an autoaugmentation flap, tucked under the breast mound to improve projection. 27 This could be either inferiorly or superiorly based, depending on the pedicle technique used, but does often require attention for exact positioning and reconstruction of the inframammary fold.…”
