Bone-turnover biomarkers as potential prognostic factors in sudden sensorineural hearing loss: A prospective cohort study

ObjectivesThis study aims to explore the relationship between bone-turnover biomarkers and the recovery of SSNHL to provide clues for further improvements in etiological research and predictors.MethodsThe medical history, hearing thresholds, biomarkers of bone-turnover, and related hormones of 117 SSNHL patients were collected prospectively between August 2018 and December 2021. Linear correlation and logistic regression models were applied to examine the association between bone-turnover biomarkers and the prognosis of SSNHL.ResultsAge, the incidence of vertigo, pure tone average of the impaired frequencies (PTAimpairedfre), and the levels of bone turnover [including alkaline phosphatase (ALP), β-carboxy terminal crosslinked telopeptide of type 1 collagen (β-CTX), and N-terminal-midfragment of osteocalcin (N-MID)] were higher in the nonresponders than responders (P < 0.05). Logistic regression showed that the age (OR = 1.035, P = 0.027), time to treatment (OR = 1.157, P = 0.038), PTAimpairedfre (OR = 1.031, P = 0.008), and β-CTX (OR = 1.004, P = 0.001) were independent risk factors for the prognosis of SSNHL. In the women SSNHL subgroup, age, postmenopause percentage, PTAimpairedfre, the activity of ALP, levels of β-CTX, and N-MID were significantly higher in the nonresponders than the responders (P < 0.05). Compared to the men SSNHL subgroup, β-CTX has a higher correlation coefficient and predictive efficiency in the women SSNHL subgroup, and logistic regression showed that β-CTX (OR = 1.004, P = 0.004) was an independent risk factor for the women SSNHL.ConclusionsBone-turnover biomarkers are risk factors for poor prognosis in SSNHL, especially β-CTX. The differences were significant in women SSNHL, which may be related to the rapid regression of estrogen after menopause that leads to the occurrence of osteoporosis with a high conversion rate.

Background: The effect of postmenopausal osteoporosis on the middle ear mechano-acoustic system is unknown. The aim of this study is to investigate whether or not middle ear resonance frequency is affected in females with postmenopausal osteoporosis. Methods: The study included postmenopausal women aged 45-60 years, separated into 2 groups as females with postmenopausal osteoporosis and healthy postmenopausal females (control group). A detailed anamnesis was taken from all subjects and then the ear, nose, and throat examinations were done followed by pure tone audiometry, tympanometry, and multifrequency tympanometry tests. The groups were compared in respect of pure tone average, bone conduction threshold, RF, static admittance, and tympanometric peak pressure values. Results: The mean age of the patients was 59.2 ± 4.53 years (range, 48-65 years) in the postmenopausal osteoporosis group and 57.11 ± 4.27 years (range, 48-65 years) in the control group ( P > .05). The mean resonance frequency values for the postmenopausal osteoporosis and control group were 954.41 ± 127.47 and 935.29 ± 126.39 Hz ( P > .05 ). The mean static admittance values for the postmenopausal osteoporosis and control group were 0.82 ± 0.33 and 0.85 ± 0.3 mmho, and mean tympanometric peak pressure values were −7.35 ± 18.52 and −6.94 ± 19.52 daPa ( P > .05 for both static admittance and tympanometric peak pressure). The mean pure tone averagevalues for the postmenopausal osteoporosis and control group were 20.96 ± 6.82 and 15.60 ± 7.81 dB, and mean bone conduction threshold values were 17.57 ± 6.03 and 12.10 ± 6.52 dB ( P < .05 for both pure tone average and bone conduction threshold). Conclusions: The results showed that the middle ear resonance frequency values were not affected in postmenopausal osteoporosis patients, but there was seen to be greater sensorineural hearing loss in females with postmenopausal osteoporosis compared to healthy postmenopausal females.

Aim: Hearing loss and osteoporosis are common geriatric syndromes. Evidence suggests that osteoporosis may have an effect on cochlear function in a small number of clinical trials. Here, cochlear function in osteoporosis patients was assessed by otoacoustic emission test (OAET) measurements. Methods: The study designed as cross-sectional and observational. Forty female patients were included in the postmenopausal period at the age of 40-75 years. Age, body mass index (BMI), vitamin D level were recorded in all patients. Audiometric threshold testing was used to measure air-and bone-conduction hearing sensitivity. Bone mineral density (BMD) of the hip and vertebra was measured using dual-energy X-ray absorptiometry (DEXA). According to vertebra L1-4 t score <-2.5 osteoporosis, -2.5 to -1 osteopenic, the group divided into two and all parameters were compared. Transiently evoked (TE), and distortion-product (DP) otoacoustic emissions were recorded. Results: The mean age of the whole group was 58.6 ± 7.9 years. Accordingly, TE left was significantly different in the higher frequency in the osteopenic group at 0.75 Hz (p = 0.015). In audiometric tests, only the osteopenic group at 6,000 Hz was significantly different in the higher frequency of both ears (p = 0.049 / p = 0.016). When the group divided into two according to femur t score <-1.0; TEright_3.5 (p = 0.04), TEright_overall (p = 0.030), TEleft_1.7.5 (p = 0.043) and TEleft_overall (p = 0.046), DPright_1 (p = 0.049) and DPleft_6 (p = 0.039) were observed to occur at higher frequencies in the osteoporotic group. Lomber t score was positively correlated with BMI (p = 0.042 / r = 0.288). BMI was lower in the osteoporotic group. The tympanogram results of all patients were Type A. The TE positivity rate (S / N> 3) was 60.8% and the DP positivity rate (S / N> 3) was 39.2%. Conclusion: According to hip BMD scores, osteopenic-osteoporotic (T scor <-1) group showed higher frequencies in both cochlear and hearing tests than normal subjects. The high frequencies in both OAET results and odiologic data in osteoporotic group support the adverse effect of osteoporosis on cochlear and hearing function. Individuals with hearing loss should be screened for osteoporosis.
