Atypical Cavernous Sinus Thrombosis: A Diagnosis Challenge and Dilemma

Abstract: Cavernous sinus thrombosis (CST) is an ocular emergency because of its devastating effect and it is prone to cause serious complications. Diagnosis of cavernous sinus thrombosis is a challenging task despite medical advancement. Efforts to promptly diagnose and initiate treatment require a high index of suspicion and a deep understanding of the disease. Unfortunately, patients suffering from CST may not always present with typical symptoms, thus making diagnosis all the more challenging. We would like to descr… Show more

“…[6,7] A non-contrast CT scan of the head may show several abnormalities such as vascular edema, protrusion of the lateral margin of the cavernous sinus, exophthalmos, and the presence of sphenoid or ethmoid sinusitis, or mass lesions near the sphenoid or pituitary gland. [4] There were signs of inflammation in an increase in leukocytes, CRP, ESR, and D-dimer on laboratory examination. On examination of pus culture, bacterial growth was found.…”

“…Cavernous sinus thrombosis is a complication of infection around the face or paranasal sinuses with a high morbidity and mortality rate. [4] The rare incidence and overlapping complaints of eye disease are generally at risk of delaying diagnosis and resulting in increased morbidity. [5] Cavernous sinus thrombosis is generally caused by septic infection, including infections around the face, especially within the "triangle of the face," including abscesses or cellulitis, dental sinusitis, otitis media, and mastoiditis.…”

“…Despite the reports on the occurrence of fungal sphenoid sinusitis leading to cavernous sinus thrombophlebitis, a wide literature search did not yield any publication on mucormycosis in an immunocompetent patient without diabetes. The anatomical importance of the sphenoid sinus is due to its proximity and location between both cavernous sinuses [ 6 ]. The proposed mechanism involves hematogenous spread causing the fungi to adhere to the endothelium of neighboring blood vessels, resulting in thrombus formation.…”

“…Esse dado é relativamente preocupante, pois as infecções odontogênicas complexas são apontadas como um problema relevante de saúde pública 14 . Nos estágios onde essa infecção atinge órgãos importantes, como o coração e o cérebro, o paciente pode ter um quadro de envolvimento das vias aérea e trombose do seio caverno, podendo causar até a morte 15 .…”
