Atypical atrial myxoma pending to the left atrial appendage: a diagnostic challenge

Abstract: DESCRIPTIONA 53-year-old woman with a known history of hypertension and hypercholesterolaemia, under oral contraception, was admitted to the neurology department due to a lacunar stroke. She presented with a right motor deficit and dysarthria, and recovered in 48 h. The cranial CT scan did not identify acute lesions. The ECG showed sinus rhythm. A transthoracic echocardiography was performed and no significant changes were found. On further evaluation, a transoesophageal echocardiography revealed an oval and p… Show more

“…The most common site of attachment is at the border of the fossa ovalis, although they can also originate from the other atrial walls. A PubMed search reveals at least 11 case reports of LAA myxomas, of which three distinctly originated from the Coumadin ridge . “…A PubMed search reveals at least 11 case reports of LAA myxomas, of which three distinctly originated from the Coumadin ridge. [3][4][5] On imaging, a myxoma appears globular or spherical, with a friable surface and heterogeneous internal echogenicity. 6 The presence of a mobile mass attached to the endocardial surface by a stalk favors the diagnosis of cardiac myxoma, although for myxomas originating within the body of the LAA, a broader base seems more typical.…”
