Atypical antipsychotics and hyperglycemic emergencies: Multicentre, retrospective cohort study of administrative data

Abstract: Risk for hyperglycemic emergencies is low after initiation of antipsychotics, but patients with pre-existing diabetes may be at greater risk. The risk appeared lower with the use of quetiapine in older patients, but the clinical significance of the findings requires further study.

“…24 We found no studies of hypoglycemia and antipsychotic treatment, but an earlier study reported that the risk of hyperglycemia, instead of hypoglycemia, in older patients with DM was highest early during therapy with atypical or conventional antipsychotics. 18 Our results and the results of Lipscombe et al 19 indicate that although few people need to be hospitalized for glucose dysregulation, some individuals, especially those with existing DM, do.…”

“…The limited evidence available indicates that the use of antipsychotics by older patients with DM is associated with an increased risk of hospital admission for hyperglycemia. 18,19 Some case reports have been published about the incidence of hypoglycemia, the counter effect of hyperglycemia during antipsychotic use. 20e27 Only three of these reports concerned older patients, one of which described a younger patient with DM.…”

“…Fasting and 2-h postprandial blood glucose did not change significantly after treatment ( P > 0.05); also, in the research of Navari,[24] it was showed that olanzapine when used over a period of months had an association with the onset of diabetes mellitus, but these effects had not been seen with short-term use of daily doses of <1 week; in the research of Lipscombe et al .,[31] it was presented that hyperglycemic emergencies such as ketoacidosis were rare (1–2 in each 1000 patients per year) in the patients receiving atypical antipsychotic drugs and were more frequent in the patients who had medical history of diabetes mellitus. In our study, no person had the history of diabetes mellitus; also, the patients were followed up only 2 months after the treatment.…”

“…Distributed data networks are being created to enable evaluation of rarer adverse drug effects across very large populations. For example, the Canadian Network for Observational Drug Effects Studies (CNODES) is a pan-Canadian collaboration of researchers that provides information regarding drug safety by combining data from 9 populationbased databases using standardized methods (13,20,21).…”
