BackgroundThis study was to estimate the incidence rate of cleft lip and/or cleft palate (CL/P) in Taiwan from 1994 to 2013, and to assess the time trend over these years.MethodsRetrospective data analysis was performed on records of all newborns with CL/P treated at Chang Gung Craniofacial Center, the only treatment center for CL/P in Taiwan, from 1994 to 2013. Three-year moving average rates were computed and linear regression was used to explore the annual average percentage change.ResultsFrom 1994 to 2013, 7282 newborns with CL/P were identified, corresponding to an annual rate of 1.48‰ (95% confidence interval (CI) = 1.45‰–1.52‰). There was a significant decline of rate of cleft lip with or without cleft palate (CL ± P) (−2.9% ± 0.2%, p < 0.0001) but slightly increase of rate of cleft palate (CP) only (+0.2% ± 0.07%, p = 0.004).ConclusionFrom 1994 to 2013, the annual rate of incidence of CL/P was 1.48‰ in Taiwan. The 2.9% annual decline of the rate was mainly from the CL ± P group, not the CP group.

Nonsyndromic orofacial clefts (NSOFCs) are the most common congenital defects in the oral and maxillofacial regions. It is mainly diagnosed prenatally through fetal ultrasonography. However, the accuracy of ultrasonography for NSOFC is unreliable. Maternal serological screening is a noninvasive method for the diagnosis of fetal malformations. In our study, we sought to identify specific biomarkers in maternal serum for predicting NSOFC prenatally. We quantified the alterations in maternal serum protein profiles between 20 pregnant women with NSOFC fetuses and 20 pregnant women with healthy fetuses by using isobaric tags for relative and absolute quantitation‐based mass spectrometry (MS). The serum levels of 75 elevated and 50 decreased proteins in the NSOFC group were detected. Twenty‐eight candidate biomarkers were selected for further confirmation by multiple reaction monitoring‐MS; of these, 16 proteins were found to be significantly different. More importantly, the levels of three proteins (APOA, HPT, and CRP) were verified by ELISAs to be obviously altered in serum from pregnancies carrying fetuses with NSOFC. Our results indicate that analysis of the maternal serum proteome is a feasible strategy for biomarker discovery of NSOFC, and APOA, HPT, and CRP proteins are potential serum biomarkers for prenatal diagnosis of NSOFC.

BACKGROUND Cleft lip and/or palate surgery is often times not accessible to all patients in developing countries, like Indonesia and other countries alike. Therefore, it is common to have the care of these patients becoming dependent upon charity care and events. Despite these efforts, there has been criticism that the performance and outcome have been suboptimal and unmonitored. The objectives of this study are-1) To identify a novel solution for resolving such issue for follow up and 2) To monitor the outcome by using Visual Analog Scale scoring system reported by the parents. MATERIALS AND METHODS Parents were separately asked to provide assessment using Visual Analogue Scale score from 1 to 10 on the appearance of their children before surgery, immediately after surgery, and six months later. Setting and Design-This is a prospective cohort study at our charity event in Gunungkidul, D. I. Yogyakarta (a small subset of the provincial district in Java Island) on January 14 th 2017. Statistical Analysis-The result was analysed using Kolmogorov-Smirnov distribution test and Friedman test. RESULTS Thirty-five parents whose 20 children underwent cheiloplasty and 15 palatoplasty were enrolled as respondents. The VAS score for before, immediately after, and six months after cheiloplasty were 3.4 with 95% CI (2.79-4.01), 7.95 with 95% CI (7.33-8.57), 8.75 with 95% CI (8.27-9.23) and P-value <0.01. The VAS score for before and immediately after, and six months after palatoplasty were 4.33 with 95% CI (3.41-5.26), 7.80 with 95% CI (7.04-8.56), and 7.12 with 95% CI (6.54-7.63) and P-value <0.01. CONCLUSION The outcome of cleft lip and palate patients in non-plastic-surgical hospital based setting showed a decent result by a significant increase in Visual Analog Scale score appraised directly by parents.
