Attenuated Salivary Cortisol Secretion Under Cue Exposure Is Associated With Early Relapse

Abstract: Alcoholic patients who use alcohol to enhance their social contacts typically lack hypothalamo-hypophysical-pituitary-adrenocortical (HPA) reactivity in the early period of abstention. They are at an increased risk of early relapse and perhaps use alcohol to increase cortisol secretion again.

“…Blunted stress-related cortisol response in abstinent alcoholics has been previously documented (Errico et al, 1993;Bernardy et al, 1996;Lovallo et al, 2000;Junghanns et al, 2003;Adinoff et al, 2005), with lower responses in those who relapsed vs those who did not (Adinoff et al, 2005;Junghanns et al, 2005). Current findings are consistent with these data and extend them by documenting a significant negative correlation between high basal levels and lower stress-related cortisol responses.…”

“…To further explore the relationship between cortisol reactivity and relapse, the cortisol response to an alcohol cue-exposure (CE) and subsequent relapse was assessed in 32 alcohol-dependent men and women (Junghanns et al, 2005). Six CE experiments were performed over a threeweek period.…”

“…While some studies did not find a positive correlation between subjective i.e. conscious craving and relapse (Drummond & Glautier, 1994;GrĂŒsser et al, 2004;Junghanns et al, 2005;Kiefer et al, 2005;Litt et al, 2000;Rohsenow et al, 1994), others did show significant correlations (Bottlender & Soyka, 2004;Cooney et al, 1997;Heinz et al, 2005b;Ludwig & Wikler, 1974;Monti et al, 1990). In contrast, changes in physiological parameters, including neuronal activation measured with functional imaging, seem to be more closely connected to relapse (Abrams et al, 1988;Braus et al, 2001;Drummond & Glautier, 1994;GrĂŒsser et al, 2004;Rohsenow et al, 1994).…”
