Atrial Septostomy and Transplantation for Patients with Pulmonary Arterial Hypertension

Abstract: The development of specific disease-targeted therapy for pulmonary arterial hypertension has offered a major advancement in the management of patients, and these medical treatments have been shown to improve both quality and quantity of life. Nevertheless, many patients deteriorate with time and will require consideration of other approaches. This article focuses on the roles of atrial septostomy and lung transplantation.

“…This pragmatic approach is supported by our review of the published experience and by similar analyses performed by others [46,47,52]. Patients with a right atrial pressure of >20 mmHg have a significantly increased risk of developing severe hypoxemia and statistically have an approximately tenfold increased risk of death within 24 h of the procedure.…”

“…Some of the deaths occurred hours after the procedure and therefore we decided to investigate the 24-h mortality. Some investigators have opted for 30-day mortality [46,47], but we chose not to do so because some critically ill patients needing AS have a high risk of A high right atrial pressure, low cardiac index, and a high pulmonary vascular resistance index and mean pulmonary arterial pressure were identified as significant risk factors for early mortality ( • Left ventricular end-diastolic pressure >18 mmHg [47,52];…”

“…Although atrial septostomy may be appropriate to consider in select patients refractory to maximal medical therapy, the procedure lacks robust study in randomized clinical trials, with data limited to single-center experiences. [16][17][18][19] Perhaps more important is the concept that PC can coexist with aggressive PAH treatment, including interventional and surgical procedures, and may be adjunctive to symptom control, particularly when advanced therapies are involved. 15,20,21 In our study, we aimed to identify physician-perceived potential barriers to PC involvement in the care of patients with PAH.…”

“…Beyond medical therapy consisting mainly of prostacyclin analogues and phosphodiesterase type 5 (PDE‐5) inhibitors, atrial septostomy (AS) has been used as a high‐risk therapeutic option in symptomatic, drug‐refractory patients with pulmonary hypertension . However, the precise role of AS in the treatment of pulmonary hypertension remains uncertain because only small series have been reported.…”
