Atrial Septal Defect with Mitral Stenosis in a Woman of 81

Abstract: SummaryFour patients suffering from virilism due to adrenocortical hyperplasia were treated with cortisone, hydrocortisone acetate, prednisone, or prednisolone for periods ranging from six to eighteen months. Oral cortisone was effective in reducing to normal the 17-ketosteroid excretion in the two younger patients, aged 31 and 16 years, but was ineffective in the two older patients, aged 19 and 20 years.Prednisone and prednisolone were the most active in depressing the 17-ketosteroid levels, prednisone being … Show more

“…7 Survival to advanced age has also been reported in another instance; an 81 years old woman experienced no symptoms related to her heart disease until she reached 75 years of age. 13 These favorable reports, however, should not obscure the fact that the long-term natural history of ASD is unfavorably influenced by MS, which augments the left to right shunt and predisposes to atrial fibrillation and right ventricular failure. 14 So, if patient is diagnosed at late stage, pulmonary hypertension and heart failure develops and the prognosis is bad.…”

“…1 In one instance in an 81 year old experienced no symptoms related to heart disease until she reached 75 years of age. 4 The hemodynamic effects of this syndrome are a result of the interplay between the relative effects of atrial septal defect and mitral stenosis. Mitral stenosis augments the left to right shunt through the ASD.…”
