OBJECTIVE: The aim of this study was to evaluate the relationship between red blood cell distribution width (RDW) and mean platelet volume (MPV) with development of postoperative atrial fi brillation (PoAF) after offpump coronary artery bypass grafting (CABG). BACKGROUND: The RDW and MPV have been associated with some cardiovascular disorders. METHODS: A total of 93 patients who underwent off-pump CABG were included in this study. The patients were divided into two groups as developing and nondeveloping PoAF groups in the postoperative period. We measured whether RDW and MPV levels are a predictive value for development PoAF. RESULTS: There were 24 patients with PoAF enrolled (mean age: 66 ± 7.8 years) and 69 patients without PoAF (mean age: 56.26 ± 11.53 years). The PoAF was signifi cantly correlated with age (p = 0.004), hematocrit (p: 0.010), RDW (p = 0.007) and creatinine (p = 0.006). Only advanced age (p = 0.012) was identifi ed as an independent predictor of PoAF. For predicting PoAF, there was 79.2 % sensitivity and 65.2 % specifi city for RDW (p = 0.001) and 62.5 % sensitivity and 55.1 % specifi city for MPV (p = 0.062). CONCLUSIONS: We found that RDW levels and MPV were not an independent predictor of the development of PoAF. Postoperative atrial fibrillation (POAF) is a common complication of cardiac surgery that occurs in up to 60% of patients. POAF is associated with increased risk of cardiovascular mortality, stroke and other arrhythmias that can impact on early and long term clinical outcomes and health economics. Many factors such as disease-induced cardiac remodelling, operative trauma, changes in atrial pressure and chemical stimulation and reflex sympathetic/parasympathetic activation have been implicated in the development of POAF. There is mounting evidence to support a major role for inflammation and oxidative stress in the pathogenesis of POAF. Both are consequences of using cardiopulmonary bypass and reperfusion following ischaemic cardioplegic arrest. Subsequently, several anti-inflammatory and antioxidant drugs have been tested in an attempt to reduce the incidence of POAF. However, prevention remains suboptimal and thus far none of the tested drugs has provided sufficient efficacy to be widely introduced in clinical practice. A better understanding of the cellular and molecular mechanisms responsible for the onset and persistence of POAF is needed to develop more effective prediction and interventions.

Operasyon öncesi alınan tüm önlemlere ve gelişen cerrahi tekniklere rağmen halen %20 ile %40 gibi yüksek oranlarda görülmektedir (1). Postoperatif AF gelişimi ile ilgili birçok risk faktörü ve prediktör belirlenmiştir. Önceden var olan AF öyküsü, ileri yaş, yapısal kalp hastalıkları gibi cerrahi ile ilişkisiz AF oluşumunda da etkili faktörlerin yanın-da, beta-blokerlerin kesilmesi, uzamış operasyon ve kardiyopulmoner baypas (KPB), sağ koroner arter tutulumu, intraaortik balon pompası (İABP) kullanımı gibi cerrahi ile ilişkili faktörler bunlardan bazılarıdır (2, 3).Atriyal ileti ve refrakterlik özelliklerindeki değişimlerin AF oluşumu ile ilişkili olduğu gösterilmiştir (4, 5). Atriyumlarda heterojen elektriksel iletim ve heterojen refrakterliğin göstergesi olan ve en uzun ile en kısa P dalga süreleri arasındaki fark olarak tanımlanan P dalga dispersiyonunun (PDD) değişik hasta popü-lasyonlarında gerek AF oluşumunu gerekse de kardiyoversiyon sonrasında AF rekürrensini öngörmede kullanılabileceğine dair yayınlar vardır (4, 6, 7). Öte yandan PDD'nin hesaplanmasında kullanılan en uzun P dalga süresinin bozulmuş interatriyal ileti ile ilişkili olduğu bilinmektedir (6). Paroksismal AF'li hastalarda bozulmuş interatriyal ileti ve uzamış interatriyal ileti süresi gös-terilmiş olmakla birlikte bu parametrenin değişik hasta popülas-yonlarında AF gelişimini öngörücü gücü konusunda yeterli bilgi yoktur (5, 8). Bu nedenle bu çalışmada, daha önce paroksismal AF gelişimini öngördüğü çeşitli çalışmalarla gösterilen ikincil değişken PDD'ye ilave olarak PDD'nin oluşumuna katkıda bulunan ve üzerinde yeterince çalışılmamış olan birincil değişken İAİS ile koroner arter baypas cerrahisi (KABC) sonrası AF gelişi-mi arasındaki ilişki araştırılmıştır. Yöntemler Hasta popülasyonuHastanemize elektif KABC için yatan ve sinüs ritminde olan 81 hasta bu prospektif, gözlemsel çalışmaya alındı. Atriyal fibrilasyon öyküsü, ileti sistemi hastalığı, ritim üzerine etkili endokrin hastalığı, kronik böbrek yetmezliği, orta-ciddi kapak hastalığı ve ciddi hipertrofi gibi ek yapısal kalp hastalığı olan, ejeksiyon fraksiyonu %55' in altında, elektrolit dengesizliği olan, beta-bloker dışı antiaritmik ilaç kullanan, KABC dışı ek bir cerrahi uygulanacak olan ve acil operasyona alınan hastalar çalışmaya alınmadı. Olguların preoperatif dönemde kullanmakta oldukları statin, beta-bloker ve anjiyotensin dönüştürücü enzim inhibitörü / anjiyotensin reseptör blokeri (ACEİ/ARB) gibi ilaçlar postoperatif dönemde de hastanın durumuna uygun şekilde devam edildi.
