This study presents estimates of life expectancy free of anxiety/depression (a health expectancy) for the adult population in Argentina and observes its evolution during the 2005-2018 period. Given that life expectancy and health expectancy are strongly correlated indicators, we decomposed the change of the latter over time to establish not only which component causes its variation, but also which age groups contributed the most to this change. We analyzed cross-sectional data of the presence of anxiety/depression symptoms using multiple waves (2005, 2009, 2013 and 2018) of the Argentine National Risk Factor Survey (ENFR) and computed a series of anxiety/depression free life expectancies (conditional on those who reached age 20), by using the Sullivan method. Later we applied a stepwise algorithm decomposition technique to determine if the change in the health expectancy was due to a mortality change or a morbidity change. We obtained that the health expectancies increased between 2005 and 2013, and decreased for 2013-2018. This variation was larger for females, where a change in morbidity was responsible for almost all improvements before 2013. The decomposition also suggested that the net impact on morbidity would be larger if not for an increase in overall life expectancy between 2013-2018, with a one-year decline in health expectancy for males and 1.7 years for females, driven mainly by the younger age groups. It is not clear yet if such negative change is transitory or the beginning of a new trend in mental health for the Argentinian population.

Este trabajo tiene por objetivo identificar los principales instrumentos de medición de estados de salud y las metodologías de valoración de preferencias que se utilizan para calcular los años de vida ajustados por calidad —AVAC—. A partir de una revisión de literatura con una estrategia de búsqueda equilibrada para la identificación de referencias bibliográficas relevantes, se presentan las características técnicas de los sistemas de clasificación en salud, como lo son los conceptos funcionales —sociales, mentales, físicos y sensoriales—, la combinación de atributos, el número de estados de salud diferentes, la duración de la encuesta y su administración. Asimismo, referente a las metodologías de valoración de preferencias, se resumen las particularidades sobre la consistencia con la teoría de los AVAC, la flexibilidad en la descripción de estados de salud, los estados peores que la muerte, entre otros aspectos. Se concluye que no existe un sistema de clasificación de estados de salud o método de valoración de preferencias mejor que otro, por lo que el investigador debe elegir la herramienta que mejor se adapte a la condición bajo análisis y al diseño de estudio en específico.

Purpose: About 12,000 people are diagnosed with lung cancer (LC)each year in Argentina, and the diagnosis has a significant impact on the whole family. The objective of this study was to characterize the health-related Quality of life (HRQoL) and the economic impact in patients with LC and in their households. Methods: We undertook an observational cross-sectional study. Outcomes included the EuroQol EQ-5D-3L (health-related Quality of life), COST (financial toxicity), WPAI-GH (productivity loss), and out-of-pocket expenses in adults diagnosed with LC at two referral public health care centers In Argentina. Results: We included 101 consecutive patients (mean age 67.5 years; 55.4% men; 57.4% with advanced disease -stage III/IV-). The mean EQ-VAS was 68.8 (SD:18.3), with 82.2% describing fair or poor health. The most affected dimensions were anxiety/depression, pain, and activities of daily living. Patients reported an average 59% decrease in their ability to perform regular daily activities as measured by WPAI-GH. Around 54.5% decreased income due to the disease, with 19.8% of patients reporting losing their jobs. The economic productivity loss was estimated at USD 2,465 person/year. About 70.3% of patients reported some degree of financial toxicity. The average out-of-pocket (OOP) expenditure was USD 100.38 person/month and represented 18.5% of household income, and the catastrophic expenditure was present in 37.1% of households. All outcomes were worse in patients with advanced disease. Conclusions Patients with NSCLCtreated in public hospitals in Argentina have significant health-related quality of life and economic impact, worsening in patients with advanced disease.
