Ataxia and cranial neuropathies from subcutaneously injected elemental mercury

Abstract: CONTEXT. Although neurological toxicity from elemental mercury vapor and organic mercury exposure has been commonly reported in the literature, it is rarely reported from soft tissue injection of elemental mercury. We present a case of neurological dysfunction from subcutaneous injection of elemental mercury. CASE DETAILS. A 35-year-old Latin American man subacutely developed gait ataxia, diplopia, and vomiting 1 year after subcutaneous injection of elemental mercury, a practice common in Afro-Caribbean and La… Show more

“…11,12 Existence in liquid form makes it possible for the metal to be injected subcutaneously, intradermally, intramuscularly, and intravenously. 8, [12][13][14] Following entry into the bloodstream, metallic mercury and its vapors undergo rapid oxidation to mercuric and mercurous ions which cannot cross the blood-brain barrier and bind to serum proteins to exert cumulative effect. 13 The metal and its vapors, however, readily cross the blood-brain barrier and accumulate in the brain since oxidation to mercury ions is not rapid enough to prevent their considerable uptake by the central nervous system (CNS).…”

“…29 Malkani et al have reported the development of limb and gait ataxia, dysdiadochokinesia, nystagmus, dysarthria along with multiple cranial nerve palsies from subcutaneous injection of elemental Hg ("quick silver pellets") with the cultural belief of protection against sickness. 30 A study in Brazilian gold traders exposed to Hg vapor have measured tremor frequency mainly in high-frequency windows (6.6-10.0 Hz). 31 Motor deficits in the form of tremor, incoordination and poor fine motor control, and subtle cognitive changes have been noted among dentists exposed toHg.…”

“…[53][54][55][56][57] Similarly, Malkani et al noted anti-Purkinje cell cytoplasmic-type Tr antibody positivity in a case of elemental mercury toxicity presenting with cerebellar ataxia. 30 In a developing country like India, ayurvedic drugs containing Hg are still being used abundantly and the physician should never forget to take detailed drug history when a patient presents with such phenotype.…”
