Uterine torsion is defined as a rotation of uterus more than 45 degrees along it’s long axis. Some degree of dextrorotation in the gravid uterus can be a normal finding, however rotation greater than 45 degrees along the longitudinal axis of the uterus which is described as uterine torsion is a rare pathological condition in obstetrical practice. We report a case of levotorsion of the uterus by 90 degrees wherein patient had presented with pre term labour at 35W5D POG and levotorsion was encountered and managed intraop during caesarean. Uterine torsion of 90 degrees was encountered with the round ligament and tubo ovarian complex along with the uterine artery lying anteriorly at the incision site and lower segment of uterus rotated laterally onto the right side. The surgery was handled diligently resulting in average blood loss. Blood loss during the surgery was average. After the delivery of the baby, uterus returned to anatomical position and levotorsion corrected spontaneously.

Uterine torsion is an extremely rare pregnancy-related complication. Its diagnosis is o ten di ficult due to unspecific clinical symptoms, which can be mistaken for other conditions such as placental abruption. A 23-year-old pregnant woman with gestational age of 25 weeks presented with acute abdominal pain and hypertonic uterus. Vaginal examination revealed a septated vagina, double cervix, and double uterus. Bedside sonography revealed absence of fetal cardiac activity and signs of placental abruption. The patient was emergently operated with a preliminary diagnosis of severe placental abruption in the uterus didelphys. During surgery, we noted a double uterus and the fetus had implanted in the right uterus, which was twisted 180 degrees to the le t. A ter detorsion, a low transverse hysterotomy was performed to extract the dead fetus. However, subsequently, the patient developed uterine atony. Subtotal hysterectomy and right adnexectomy were inevitable due to failure of conservative treatment with B-lynch suture. The patient's postoperative condition was stable, and she was discharged a ter 5 days.

Uterine torsion is a rare life-threatening event that happens at any age or any gestational age. By defi nition, it consists of a rotation of more than 45 degrees around the long axis of the uterus. The reported cases have variable presentations. The uterine torsion can happen without any sequelae either for the fetus or the mother. However, fetal and maternal mortalities were also reported in such a case We hereby, report the case of a 29-year-old female patient, with previous four Normal Vaginal Deliveries, pregnant with twins, presenting at 36 weeks gestation with an irreducible uterine torsion at the third trimester of her pregnancy complicated by maternal and fetal deaths. We concluded that the prognosis is improved as long as the management is done rapidly. More data is needed to know about the genetic predilection and the characteristics of imaging workup for a rapid preoperative diagnosis of this condition.
