BackgroundPsoriasis is associated with an increased mortality risk, with cardiovascular disease being the leading excess cause (in a dose–response manner with psoriasis severity). Statins have demonstrated a reduction in all‐cause mortality with no excess of adverse events among the general population. The underuse of interventions in cardiovascular prevention, such as statins, for patients with psoriasis may be the result of an insufficient evaluation.ObjectivesTo provide the dermatologist with a tool for systematizing the treatment of dyslipidemia in psoriasis, which generally escapes the scope of dermatological practice, and to facilitate decision‐making about the referral and treatment of patients.MethodsThe Psoriasis Task Force of the European Academy of Dermatology and Venereology performed this two‐phase study to achieve a consensus and create recommendations on the use of statin therapy in patients with psoriasis. The first phase included a systematic review to identify a list of outline concepts and recommendations according to guidelines. The second phase consisted in a two‐round Delphi study to evaluate those recommendations not literally taken from guidelines.ResultsA list of 47 concepts and recommendations to be followed by dermatologists involved in the treatment of patients with moderate–severe psoriasis was created. It included six main concepts about cardiovascular risk and psoriasis, six items related with the role of low‐density lipoprotein cholesterol (LDL‐c) and the benefits of statin treatment in psoriasis patients, eight recommendations about how cardiovascular risk should be assessed, three on the role of non‐invasive cardiovascular imaging, three on LDL‐c thresholds, eight key points related to statin prescription, 10 on statin treatment follow‐up and three on patient referral to another specialist.ConclusionsThe application of this position statement (close final list of concepts and recommendations) will help dermatologists to manage dyslipidemia and help psoriasis patients to reduce their cardiovascular risk.

The effectiveness and safety of biological agents for treating psoriasis have been confirmed; however, their effects on glucose metabolism biomarkers in psoriasis patients remain unclear. A systematic review and meta‐analysis were performed according to PRISMA guidelines. The final analysis enrolled 12 studies, including eight randomized controlled trial (RCT) (n = 5628 patients) and four observational cohort studies (OBSs) (n = 393 patients). The meta‐analysis comprising nine studies (six RCTs and three OBSs) revealed a slight reduction in the levels of HOMA‐IR associated with the use of biological therapies in OBS (biological therapies vs. traditional therapies: WMD = −0.2, CI = −0.10 to 0.50, p = 0.02). Although a considerable number of studies were analysed, our review did not show a significant alteration in HOMA‐IR levels among patients treated with biological therapies such as IL‐17 inhibitors and IL‐12/23 inhibitors at weeks 12–16 in RCTs. We also did not observe remarkable alterations in the fasting plasma glucose levels of patients in both OBS and RCT. Additional RCT on a larger scale and duration is required to provide more conclusive evidence regarding the effect of biological agents on glycogen metabolism in psoriasis.

BackgroundIncreasing evidence suggests that insulin resistance is linked to cardiovascular disease and atherosclerosis. The triglyceride–glucose (TyG) index has proven to be a convincing marker to quantitatively evaluate insulin resistance. However, there is no relevant information about the relationship between the TyG index and restenosis after carotid artery stenting.MethodsA total of 218 patients were enrolled. Carotid ultrasound and computed tomography angiography were used to evaluate in-stent restenosis. A Kaplan–Meier analysis and Cox regression method were performed to analyze the correlation between TyG index and restenosis. Schoenfeld residuals were used to determine the proportional-hazards assumption. A restricted cubic spline method was used to model and visualize the dose–response relationship between the TyG index and the risk of in-stent restenosis. Subgroup analysis was also performed.ResultsThirty-one participants (14.2%) developed restenosis. The preoperative TyG index had a time-varying effect on restenosis. Within 29 months post-surgery, an increasing preoperative TyG index was linked to a significant increased risk of restenosis (hazard ratio: 4.347; 95% confidence interval 1.886–10.023). However, after 29 months, the effect was decreased, although not statistically significant. The subgroup analysis showed that the hazard ratios tended to be higher in the age ≤ 71 years subgroup (p < 0.001) and participants with hypertension (p < 0.001).ConclusionThe preoperative TyG index was significantly associated with the risk of short-term restenosis after CAS within 29 months post-surgery. The TyG index may be employed to stratify patients based on their risk of restenosis after carotid artery stenting.
