Association of Systemic Inflammation and Sarcopenia With Survival in Nonmetastatic Colorectal Cancer

Abstract: Prediagnosis inflammation was associated with at-diagnosis sarcopenia. Sarcopenia combined with inflammation nearly doubled risk of death, suggesting that these commonly collected biomarkers could enhance prognostication. A better understanding of how the host inflammatory/immune response influences changes in skeletal muscle may open new therapeutic avenues to improve cancer outcomes.

“…recommend obtaining and documenting inflammatory status (CRP and albumin) . The N/L ratio has been linked to different degrees of SMM loss . However, in this study, the serum levels of CRP were available for only 24.5% of the patients for whom the CAX stage was evaluable and could not include it as a staging criteria.…”

“…Body circumference was measured at the umbilicus (waist) and greater trochanter (hip). Muscle and fat cross‐sectional areas were normalized by dividing values by the body surface area (BSA) calculated as the height of the patient squared . Body mass index (BMI) and waist‐to‐hip ratio (WHR) was calculated using standard equations, and patients who had a BMI ≥ 30 kg/m 2 were defined as obese according to World Health Organization (WHO) criteria…”

“…Sarcopenia was defined by stratifying for sex and obesity status and using optimal stratification methods for psoas muscle area that have been previously described and validated . Specific sarcopenic cutoff values were defined using the maximal predictive value for death (Youden’s index) on receiver operating characteristic (ROC) curves for each group.…”

“…7 Several studies suggest that systemic inflammation may lead to ongoing muscle loss in cancer patients and in part drive the associations with cancer survival. 8,9 In vitro studies also highlight the key role of inflammation. For example, exercise-induced myokines released from skeletal muscle, such as interleukin-6, suppress tumor growth.…”
