The intraoperative activity of a surgeon has substantial impact on postoperative outcomes. However, for most surgical procedures, the details of intraoperative surgical actions, which can vary widely, are not well understood. Here we report a machine learning system leveraging a vision transformer and supervised contrastive learning for the decoding of elements of intraoperative surgical activity from videos commonly collected during robotic surgeries. The system accurately identified surgical steps, actions performed by the surgeon, the quality of these actions and the relative contribution of individual video frames to the decoding of the actions. Through extensive testing on data from three different hospitals located in two different continents, we show that the system generalizes across videos, surgeons, hospitals and surgical procedures, and that it can provide information on surgical gestures and skills from unannotated videos. Decoding intraoperative activity via accurate machine learning systems could be used to provide surgeons with feedback on their operating skills, and may allow for the identification of optimal surgical behaviour and for the study of relationships between intraoperative factors and postoperative outcomes.

Background The utilization of simulation in healthcare education has grown significantly in the last 20 years. Although typically used for training purposes, simulation is increasingly used for assessing technical skills needed in clinical practice. Despite its common use, there has been no investigation into the full potential of simulation in healthcare education. This study aims to explore the scope of simulation-based assessments in healthcare education. Methods Using the Joanna Briggs Institute’s scoping review methodology, we searched Ovid MEDLINE, Embase, Scopus, WOS, CINAHL, APA PsycINFO, and Embase databases and two well-known simulations in medical education, Clinical Simulation in Nursing and Simulation in Healthcare, from 2021 to 2023. We also checked the reference lists of review articles. We imported, deduplicated, and screened the search results in EndNote X20. We included 49 studies that met our criteria and analyzed them descriptively and narratively. Results We specified the target groups and applications for simulation-based assessment in healthcare professional education. We also identified challenges, advantages, and disadvantages, as well as the necessary conditions for conducting such assessments. Conclusions Simulation-based evaluation is beneficial for health education in evaluating competence and safety. Nevertheless, limitations such as high costs and low realism should be taken into account. Stakeholders and educators must study these concerns, establish standards for evaluation and sampling, and conduct planning and evaluation before using these methods.

Background Maternal-fetal attachment (MFA) has been reported to be associated with the postpartum mother-infant relationship. Seeing the fetus through ultrasound might influence MFA, and the effect could be increased by more realistic images, such as those generated in virtual reality (VR). Objective The aim was to determine the effect of fetal images generated in VR on MFA and depressive symptoms through a prenatal-coaching mobile app. Methods This 2-arm parallel randomized controlled trial involved a total of 80 pregnant women. Eligible women were randomly assigned to either a mobile app–only group (n=40) or an app plus VR group (n=40). The VR group experienced their own baby’s images generated in VR based on images obtained from fetal ultrasonography. The prenatal-coaching mobile app recommended health behavior for the pregnant women according to gestational age, provided feedback on entered data for maternal weight, blood pressure, and glucose levels, and included a private diary service for fetal ultrasound images. Both groups received the same app, but the VR group also viewed fetal images produced in VR; these images were stored in the app. All participants filled out questionnaires to assess MFA, depressive symptoms, and other basic medical information. The questionnaires were filled out again after the interventions. Results Basic demographic data were comparable between the 2 groups. Most of the assessments showed comparable results for the 2 groups, but the mean score to assess interaction with the fetus was significantly higher for the VR group than the control group (0.4 vs 0.1, P=.004). The proportion of participants with an increased score for this category after the intervention was significantly higher in the VR group than the control group (43% vs 13%, P=.005). The feedback questionnaire revealed that scores for the degree of perception of fetal appearance all increased after the intervention in the VR group. Conclusions The use of a mobile app with fetal images in VR significantly increased maternal interaction with the fetus.
