Association of Surgical Resection, Disability, and Survival in Patients with Glioblastoma

Abstract: Objective Extent of resection (EOR) and Karnofsky Performance Status (KPS) are at odds in glioblastoma (GBM) surgery, that is, the anticipated postoperative disability limits the EOR. This study analyzes the correlation of different surgical modalities with the resulting physical status and survival of patients with GBM. Methods A total of 565 patients with primary GBM were operated on in a single institution between 2006 and 2014. Possible surgical modalities comprised supratotal resection (SLR), … Show more

“…For example, the extent of resection of the enhancing part of a glioblastoma correlates with the survival time of the patients. [2][3][4][5] Unfortunately, conventional MRI tends to underestimate the extent of diffuse infiltrative glioma growth, as the infiltrating cells' spread exceeds the enhancing part of the tumor. 6,7 It is known that glioma cells migrate into the tumor edema, which appears hyperintense on T2-weighted (T2w) and FLAIR images, or even beyond.…”

“…Apart from the diagnostic value, identification of tumor regions enhancing after CA administration has implications for treatment. “…In the study of Ahmadipour et al, the HR of biopsy compared with total resection was 2.33 (95%CI 1.77-3.06) for death. 17 Therefore, we calculated a sample size of 26 patients per group at 80% power and with an α level of 0.05, using the Freedman method. 18 Propensity score regression adjustment was used to run the outcome model of the association between EOR and survival controlled by PS and posttreatment variables from the unmatched dataset.…”

“…In many of these cases, in fact, Temozolomide is given as single treatment. Withdrawal of radiation may also contribute to the poor prognosis associated with multifocal glioblastoma [40,41].…”
