Type 2 diabetes (T2D) is associated with an increased risk of fracture, which has been reported in several epidemiological studies. However, bone mineral density in T2D is increased and underestimates the fracture risk. Common risk factors for fracture do not fully explain the increased fracture risk observed in patients with T2D. We propose that the pathogenesis of increased fracture risk in T2D is due to low bone turnover caused by osteocyte dysfunction resulting in bone microcracks and fractures. Increased levels of sclerostin may mediate the low bone turnover and may be a novel marker of increased fracture risk, although further research is needed. An impaired incretin response in T2D may also affect bone turnover. Accumulation of advanced glycosylation endproducts may also impair bone strength. Concerning antidiabetic medication, the glitazones are detrimental to bone health and associated with increased fracture risk, and the sulphonylureas may increase fracture risk by causing hypoglycemia. So far, the results on the effect of other antidiabetics are ambiguous. No specific guideline for the management of bone disease in T2D is available and current evidence on the effects of antiosteoporotic medication in T2D is sparse. The aim of this review is to collate current evidence of the pathogenesis, detection and treatment of diabetic bone disease.

Diabetic kidney disease comprises a multifaceted etiopathology that includes glomerular hemodynamic changes, inflammation, oxidative stress, interstitial fibrosis, and tubular atrophy. Diabetic nephropathy (DN), or the pathological alterations in the capillaries caused by diabetes mellitus (DM), is the major cause of end-stage renal disease in diabetic individuals worldwide. Proteinuria, among other elements, as a diagnostic biomarker for DN, is a late change, and so does not detect controlled early nephropathic alterations. This needs the development of novel biomarkers that are more sensitive, specific, and early than proteinuria. Endothelial dysfunction and atherosclerosis are caused by DM, whereas glomerular hypertrophy, an increase in the extracellular matrix, and glomerular sclerosis are caused by DN. Osteoglycin/Osteoinductive Factor (OGN) is a secretory small leucine-rich matrix/basement proteoglycan that regulates lipid and glucose metabolic activity, collagen fibrillogenesis, and cytokine availability in a paracrine/endocrine manner, and has been linked to the development of atherosclerosis, neovascularization, and angiogenesis. OGN is a pathogenic effector and biomarker that is extremely relevant to DM and DN because of these properties. OGN appears to be a better biomarker for DN than microalbuminuria, according to research. This literature review is intended to offer the most recent findings relevant to OGN's DN biomarker potential.

Background: The increased risk of fractures in individuals with diabetes has been underestimated by conventional fracture predictors such as the bone mineral density Tscore or the Fracture Risk Assessment Tool. The present study conducted a systematic review and a meta-analysis to investigate the association between the levels of bone turnover markers (BTMs) and fractures in patients with diabetes. Methods: We conducted a systematic literature search. Eligibility criteria were studies investigating BTMs in patients with diabetes with/without fractures. For the meta-analysis, we primarily used the fixed effects model. The estimates were reported as the standardized mean difference (SMD). Results: We included eight observational studies. The levels of osteocalcin, procollagen type 1 amino terminal propeptide (P1NP), and insulin-like growth factor-1 (IGF-1) were found to be significantly lower in individuals with fracture than in those without fracture (-0.36 [-0.46,-0.26]) (SMD [95% confidence interval], (-0.57 [-0.75,-0.40]), and (-0.50 [-0.61,-0.39]), respectively. The levels of N-terminal cross-linked telopeptide of type 1 collagen (NTX), sclerostin, and bone-specific alkaline phosphatase (BAP) were significantly higher in patients with fracture (0.24 [0.13, 0.35], (0.47 [0.29, 0.65]), and (0.14 [0.01, 0.27]), respectively, whereas C-terminal cross-linked telopeptide (CTX) showed no difference between the groups. The sensitivity analysis revealed an effect of study size but not study design. Conclusions: The results suggest that BTMs could be associated with fracture status in individuals with diabetes with lower levels of formative BTMs and IGF-1 and higher levels of NTX, sclerostin, and BAP in these individuals than in those without fracture. However, some results depended on one large study.
