Alopecia areata (AA) is a T cell-mediated autoimmune disease resulting in the destruction of hair follicles. Ophiasis refers to a subtype of AA that presents as a symmetric, band-like hair loss pattern of the occipital, temporal, and parietal regions of the scalp. We present a case of a 58-year-old white man with AA, ophiasis pattern, who was treated with clobetasol 0.05% solution and four treatments of microneedling with triamcinolone over 6 months. He underwent gradual improvement, most notably on his left occipital scalp where his hair loss was most prominent, with near complete hair regrowth on his left occipital scalp. Microneedling with triamcinolone can be considered as a promising treatment in cases of ophiasis AA.

Background: Alopecia areata (AA) is an autoimmune, nonscarring hair loss disorder with slightly greater prevalence in children than adults. Various treatment modalities exist; however, their evidence in pediatric AA patients is lacking.Objective: To evaluate the evidence of current treatment modalities for pediatric AA. Methods:We conducted a systematic review on the PubMed database in October 2019 for all published articles involving patients \18 years old. Articles discussing AA treatment in pediatric patients were included, as were articles discussing both pediatric and adult patients, if data on individual pediatric patients were available.Results: Inclusion criteria were met by 122 total reports discussing 1032 patients. Reports consisted of 2 randomized controlled trials, 4 prospective comparative cohorts, 83 case series, 2 case-control studies, and 31 case reports. Included articles assessed the use of aloe, apremilast, anthralin, anti-interferon gamma antibodies, botulinum toxin, corticosteroids, contact immunotherapies, cryotherapy, hydroxychloroquine, hypnotherapy, imiquimod, Janus kinase inhibitors, laser and light therapy, methotrexate, minoxidil, phototherapy, psychotherapy, prostaglandin analogs, sulfasalazine, topical calcineurin inhibitors, topical nitrogen mustard, and ustekinumab.Limitations: English-only articles with full texts were used. Manuscripts with adult and pediatric data were only incorporated if individual-level data for pediatric patients were provided. No meta-analysis was performed. Conclusion:Topical corticosteroids are the preferred first-line treatment for pediatric AA, as they hold the highest level of evidence, followed by contact immunotherapy. More clinical trials and comparative studies are needed to further guide management of pediatric AA and to promote the potential use of pre-existing, low-cost, and novel therapies, including Janus kinase inhibitors.

Num estudo realizado por Jury et al, 13 foi avaliada a eficácia do UVB--BE em várias dermatoses nas crianças com idade inferior a 16 anos, incluindo 6 com AA, que realizaram tratamento nas áreas afetadas com média de 20 sessões, sendo que 83% não demonstraram qualquer melhoria e apenas 4 tiveram repovoamento capilar que não durou mais do que 2-3 meses.…”

Atualmente, com o desenvolvimento recente de agentes biológicos e outros novos fármacos, alguns dos quais com o seu uso já aprovado em idade pediátrica, torna-se importante contextualizar o papel da fototerapia na dermatologia. Esta tem um perfil de segurança excelente e bem reconhecido desde há décadas, para além de não necessitar de monitorização laboratorial, o que constitui uma mais valia, sobretudo na população pediátrica. Tem sido utilizada com sucesso nesta faixa etária no tratamento de diversas doenças dermatológicas. Neste artigo é realizada uma revisão acerca das principais particularidades da utilização da fototerapia em idade pediátrica, focando os efeitos adversos, os principais obstáculos, as suas indicações e a seleção individualizada das diferentes modalidades, de modo a facilitar a prática clínica desta terapêutica.
