Association of Immune-Mediated Cerebellitis With Immune Checkpoint Inhibitor Therapy

Abstract: Immune-mediated encephalitis related to immune checkpoint inhibitor therapy is a rare but increasingly described condition that can cause significant morbidity. There are several reported cases in the literature but no previously described cases of immune-mediated cerebellitis. We describe a case of acute cerebellitis that developed in a 20-year-old man with primary refractory Hodgkin lymphoma being treated with the immune checkpoint inhibitor nivolumab. After exposure to 3 cycles of nivolumab, the patient had… Show more

“…We searched PubMed for irEncephalitis (including meningoencephalitis, encephalomyelitis, and cerebellitis) cases and identified 60 patients (Tables 1 and S1), 22,32‐73 35 (58.3%) of which were males. The median onset age was 60 years (interquartile range, IQR: 53‐69; range: 19‐83).…”

“…Most of these complications occur during immunotherapy treatment. However, some rare reports of immune-mediated syndromes emerged a few months after immunotherapy stop [10, 11]. Thus, it becomes difficult to distinguish between a pure paraneoplastic syndrome and acute or late neurologic toxicity of immunotherapy, and, in the literature, there is sometimes an arbitrary use of these terms.…”

“…Cerebellar involvement can also occur, presenting sometimes as acute cerebellitis with abnormal T2 hyperintensities and contrast enhancement (62,63), and in some cases with cerebellar atrophy (30,31). Neural antibodies including ANNA-1, voltagegated calcium channel (VGCC) and neural intermediate filament (NIF) have been reported in patients presenting with cerebellar ataxia (30).…”

“…The majority of patients present a rapid progression of symptom severity. CPK is usually significantly elevated above 1,000 IU (median 680-2,668 U/L) (63,(100)(101)(102). EMG shows myopathic features (30,100,101).…”
