Association of HLA B8 with spontaneous resolution in sarcoidosis.

Abstract: HLA may influence the natural history of some diseases. HLA frequencies have been compared in 164 healthy control subjects, 50 patients with sarcoid lung fibrosis, and 37 patients with sarcoidosis that resolved spontaneously. B8 was increased significantly in the resolved group compared to both healthy control subjects (p <0001) and the fibrotic group (p <0 01). The results support the findings of other investigators, and are consistent with the hypothesis that inherited host factors, perhaps related to immune… Show more

“…We found an increased frequency of HLA-A1 and B8 markers in the 59 patients as shown in table 1 in agreement with data previously published by ourselves [31] and others [32], We also studied a possible heter ogeneity of the disease: we confirmed the correlation found between HLA-B8 and dis ease resolution (either spontaneous or pred nisone-induced), reported elsewhere [32] and also with female sex, as shown in table 1. HLA-DR5 frequency was significanctly in creased in patients as a whole but even more so in males and in treatment-resistant or relapsing patients, as has been recently de- this marker by isoelectric focusing.…”

“…A large number of HLA class I and II alleles have been reported to be overrepresented in sarcoidosis, and especially HLA-DR3 was early recognized to associate with sarcoidosis in general and in particular with acute manifestations of the disease, such as EN and ankle arthritis or periarticular inflammation (i.e., features linked to a good prognosis) (10)(11)(12)(13). Subsequent studies have corroborated a strong association with specific HLA alleles (14) (i.e., HLA-DRB1*0301), and in such patients identified a typical lung accumulation of CD4 Ï© T cells expressing a particular T-cell receptor (TCR) for antigen, AV2S3, suggesting the presence of a specific antigen (15).…”

“…The HLA-B8, DR3 haplotype has been associated with acute-onset, stage I resolving sarcoidosis [106][107][108][109] in Caucasians but not in West Indians [107]. HLA-DR5 has also been associated with sarcoidosis [103][104][105].…”
