Association of Checklist Use in Endotracheal Intubation With Clinically Important Outcomes

Abstract: IMPORTANCE Endotracheal intubation of critically ill patients is a high-risk procedure. Checklists have been advocated to improve outcomes. OBJECTIVE To assess whether the available evidence supports an association of use of airway checklists with improved clinical outcomes in patients undergoing endotracheal intubation.

“…Checklists and other decision support tools have received growing attention in medicine over the past decade as a means to reduce avoidable medical errors. Checklists have been shown to improve clinical outcomes in a wide variety of situations ( 35 , 36 ) although they may not be useful in every clinical situation ( 37 , 38 ). At the same time, some providers have resisted the implementation of checklists for various reasons; for example, providers who perceive the checklists as redundant or unhelpful are less likely to use them ( 39 ).…”

“…Implementation of the checklist was associated with rapid reductions in mortality and complications among patients who were undergoing surgery [ 27 , 28 ]. Reductions in complication rates have been reported after the introduction of RSI checklists applied to emergency intubation in the emergency department [ 29 ], and in the intensive care unit [ 30 ]. However, it is important to understand the limitations of checklist use.…”

“…They also do not necessarily modify the presence of difficult patient factors. This may account for some recent studies which report a lack of effect in some clinical endpoints after the implementation of checklists in emergency intubation practice [ 30 32 ]. A recent study in pre-hospital practice among experienced Scandinavian providers reported benefits and disadvantages in checklist use as well as highlighting considerable variation in checklist practice [ 33 ].…”

“…In health, checklists are used universally, they are characterized as a cognitive strategy that ensures adequate preparation through the checking of steps for a given procedure. They are associated with greater safety, as they provide a reduction in accidents, failures or adverse events (7)(8) , in addition to reducing errors and improving the quality of care (9) . Thus, although it is considered pertinent to use a checklist for disinfecting ambulances that transfer patients affected by Covid-19, it is pointed out the relevance of this being the result of a scientific process of construction and validation.…”
