Association of aspartate aminotransferase in statin-induced rhabdomyolysis

Abstract: INTRODUCTION The risk of rhabdomyolysis in the general population is elevated by the increased prevalence of statin use. As the presentation of rhabdomyolysis is varied, there is a risk of delayed diagnosis leading to patient complications and increased healthcare costs. Creatine kinase (CK) alone is not sufficiently predictive for risk stratification. Beyond serum CK, other biomarkers such as transaminases may be used as surrogates to evaluate rhabdomyolysis severity and predict complication risks. AIM To ass… Show more

“…In a retrospective study of 215 cases of rhabdomyolysis with a CK ≥1000 U/L, the AST was elevated in 93% cases and ALT in 75% of cases 8 . An elevated AST:ALT ratio might also support rhabdomyolysis, as the leakage of intracellular contents from skeletal muscles precipitates a favorable elevation of AST 9 . Patients should be evaluated clinically for symptoms of rhabdomyolysis, including weakness, myalgia, and darkened urine.…”

“…Myoglobin (MYO), Cr, potassium, and other muscle enzymes like aminotransferases and lactate dehydrogenase (LDH) are released as a result of myocyte injury, whereas elevated aspartate aminotransferase (AST) is linked to increased peak creatine kinase (CK) levels and severity of RM (26,27). An AST test may be requested by doctors to help with the clinical diagnosis of RM (28). For the classification of patients at risk for RM-induced AKI, serum LDH is a suitable and affordable prognostic biomarker (29).…”

“…9 From Singapore, ruan, low and Kwan show that aspartate aminotransferase (aSt) is the strongest measure to use in diagnosing rhabdomyolisis, a condition increasing in incidence with increasing statin use. 10 We always welcome work from early career GPs. Our improving Performance paper reports an early career doctor's audit of the care of obese patients in her general practice placement.…”
