Association Between Parapneumonic Effusion and Pericardial Effusion in a Pediatric Cohort

Abstract: We found a high incidence of pericardial effusions in pediatric patients with parapneumonic effusions. Leukocytosis, higher pleural fluid leukocyte and neutrophil counts, and a propensity for surgical intervention suggest a prognostic relationship between pericardial effusions and more severe parapneumonic disease. The majority of these pericardial collections resolve with treatment of the underlying pleural disease.

“…Accumulation of fluid between the heart and the pericardial sac, or pericardial effusion, is rare in childhood but may co-exist with parapneumonic effusion, particularly left-sided cases, simply due to the proximity of the pleura and pericardium [71,72]. Most of these associated pericardial effusions improve with treatment of the underlying infection but urgent echocardiography should be considered in cases of haemodynamic compromise as pericardiocentesis may be required [73].…”

“…Finally, children with parapneumonic effusion should be carefully observed for signs and symptoms of haemodynamic deterioration due to pericardial effusion 90 98 99. Although the amount of pericardial fluid is usually very small and does not give rise to symptoms, pericardiocentesis is sometimes needed.…”

“…A surprisingly high incidence of pericardial effusion concomitant with PE has been reported in the very sick child [27]. However, specific treatment is rarely needed.…”
