“…To the Editor Identification and characterization of biomarkers predictive of pathologic response after neoadjuvant therapy has emerged as a hot topic in the oncology field. 1 Shoucair et al 2 shed some light on this topic, underscoring the practical importance of matrix metalloproteinase 7 as a promising biomarker teased out through RNA sequencing when predicting pathologic response to neoadjuvant therapy for pancreatic ductal adenocarcinoma (PDAC). This finding was derived from a discovery cohort involving 23 patients, a validation cohort involving 80 patients, and an exploratory cohort involving 60 patients, yet considerable methodological concerns have been raised regarding the robustness of this study by Shoucair et al The first concern relates to the large effect size estimate (odds ratio, 21.25) with wide 95% CI (6.19-72.95) when quantifying the association of skin matrix metalloproteinase 7 with pathologic response after neoadjuvant therapy, 2 which reflects lack of statistical power; albeit, this association was reproducible in both validation and exploratory sets.…”
