Association between Helicobacter pylori Virulence Factors and Gastroduodenal Diseases in Okinawa, Japan

Abstract: The incidence of gastric cancer in Okinawa is lowest in Japan. Some previous reports using small number of strains suggested that the high prevalence of Helicobacter pylori with Western-type cagA in Okinawa compared to other areas in Japan might contribute to the low incidence of gastric cancer. It has still not been confirmed why the prevalence of Western-type cagA strains is high in Okinawa. We examined the association between the virulence factors of H. pylori and gastroduodenal diseases in Okinawa. The gen… Show more

“…Antral biopsy specimens were obtained for the isolation of H. pylori by standard culturing technique as previously described [7]. H. pylori DNA was extracted from confluent plate cultures xpanded from a single colony by using DNeasy Blood & Tissue Kit ® (Qiagen, Inc., Santa Clarita, CA).…”

“…There are data suggesting that within the same geographic area infection with east Asian-type cagA strains is associated with a higher risk of peptic ulcer or gastric cancer than infection with western-type cagA strains [2527]. For example, data from Okinawa suggested that the pattern of gastritis in those with east Asian type CagA was that of a rapid progression to atrophic gastritis compared with infections with western types of CagA [28]. However, the individuals with these different types of CagA may also differ in terms of other factors unrelated to CagA type, such as when they or their families migrated to Okinawa and diet among others.…”

“…Eastern Asian H. pylori strains have been documented as more pathogenic, correlating with an increased occurrence of gastric carcinoma (among other GI pathologies) in H. pylori -positive patients in East Asia (including the countries of Japan, Korea, China, and Russia) [41,64]. This observed increase in pathogenicity correlates with an increase in CagA phosphosite abundance [7].…”
