Association Between Diabetes and Cause-Specific Mortality in Rural and Urban Areas of China

Abstract: Importance In China diabetes prevalence has increased substantially in recent decades, but there are no reliable estimates of the excess mortality currently associated with diabetes. Objective To assess the proportional excess mortality associated with diabetes, and to estimate the diabetes-related absolute excess mortality in rural and urban China. Design, setting, and participants A 7-year nationwide prospective study of 512,869 adults aged 30-79 years from 10 (5 rural, 5 urban) localities across China, … Show more

“…20,21 An epidemiologic report from China demonstrated that the relative risk of mortality is remarkably high (relative risk, 83.3; 95% CI, 53.2-130.5) in DKD compared with that in non-DKD (relative risk, 1.72; 95% CI, 1.1-2.6). 22 Overt proteinuria is considered as the most important surrogate end point and a therapeutic target in clinical trials for prevention of DKD. 17,23 If our results are confirmed by future studies, a 3.7% decrease in absolute risk of proteinuria (or number needed to treat=27) associated with folic acid therapy could translate into ≈1.5 million of patients with hypertension and diabetes mellitus being spared from DKD during a 4.4-year period, given that ≈40 million of Chinese population currently experiences hypertensive diabetes mellitus.…”

“…DKD is also the main cause of endstage renal disease (ESRD) [2]. The incidence of DM in China has increased each year [3], and the incidence of DKD is also increasing. According to the results of the Shanghai Dialysis Registration System in 2015, DKD accounted for 14.32% of year-end ESRD patients and was ranked as the second-leading cause of ESRD in China, after chronic glomerulonephritis (42.4%).…”

“…6,7 As a disorder of glucose metabolism, diabetes mellitus affects multiple organ systems and is associated with a variety of vascular and several nonvascular complications. [8][9][10] Ongoing reliable estimations are needed to plan effective national prevention and treatment programs for diabetes management.…”
