Cardiovascular disease is the leading cause of morbidity and mortality worldwide. During the past few decades, several studies have demonstrated an association between religiosity/spirituality and beneficial effects on mental and physical health. Longitudinal studies have found significant association between regular attendance at religious services and reduced all-cause and cardiovascular mortality. Among the several potential pathways of higher religiosity/spirituality, there are biological mechanisms such as decrease the inflammatory cytokines and cortisol levels, enhance of parasympathetic activation and attenuation of sympathetic nervous system activity. Prayer can be a possible mediator of the positive religiosity/spirituality effects in the cardiovascular system. The objective of this article is to review the potential mechanisms by which prayer interferes in the cardiovascular system, and which would explain its influences in cardiac mortality and morbidity. We found that prayer was associated with a lower prevalence of coronary artery disease. Studies have demonstrated that prayer was associated with a parasympathetic activity increase and decrease in the sympathetic nervous system component. In conclusion, prayer had been linked to higher levels of cardiac autonomic, a condition that is associated with a reduced risk of cardiac arrhythmias, and cardiac mortality. Scientific research on the health benefits of prayer should be expanded, aiming to obtain a clearer understanding of the possible positive action of prayer on the cardiovascular system, especially in the cardiac autonomic modulation. Although there is no recommendation of prayer prescription by cardiologists, the pivotal effects of this spiritual practice should be known within the cardiology.

Kawasaki disease (Kd) is characterized by an acute febrile, self-limited, generalized multisystem vasculitis that typically affects children aged less than five years. 1 The underlying etiology of Kd remains unclear. nonetheless, epidemiological studies suggest that asymptomatic or non-vasculitis infection agents in genetically susceptible individuals could play a role in Kd's pathogenesis. 2,3 Patients with Kd will develop a variety of clinical manifestations. However, the most important clinical feature is cardiovascular sequels including coronary artery (Ca) aneurysm (which occurs in almost 25% of untreated cases), myocardial infarction, arrhythmias, and cardiomyopathy with depression of myocardial contractility. 1,4-6 a delay in diagnosis and initiation of appropriate treatment will result in more undesirable outcomes, such as the increased risk of Ca aneurysm. 7 In addition, injury to myocardial tissue due to excessive inflammation can cause an altered electrical impulse propagation, which manifests through the regional heterogeneity of QT interval duration in myocardial recovery times. 8 This inter-lead differences on standard 12-lead electrocardiogram (eCG) is termed QT dispersion. 9 Several studies have demonstrated that increase in QT dispersion has an important predictive value for developing potentially lethal arrhythmias and Ca aneurysms. BackgroundThe heart is commonly involved in COVID-19, and rhythm disorders have been largely reported.ObjectiveTo evaluate the association of some non-cardiac and cardiac comorbidities and QT dispersion with arrhythmias and their impact on outcomes in hospitalized patients with COVID-19.MethodsEach patient underwent cardiac telemetry monitoring through the entire hospitalization period, laboratory analyses, 12-lead ECG, and lung imaging examination. Patients with arrhythmia were divided into three groups (bradyarrhythmias, tachyarrhythmias, and tachy- and bradyarrhythmias).ResultsTwo-hundred patients completed the study (males, 123; mean age, 70.1 years); of these, 80 patients (40%) exhibited rhythm disorders on telemetry. Patients with arrhythmia were older (p < 0.0001), had a greater number of comorbidities (p < 0.0001), higher values of creatinine (p = 0.007), B-type natriuretic peptide (p < 0.0001), troponin (p < 0.0001), C-reactive protein (p = 0.01), ferritin (p = 0.001), D-dimer (p < 0.0001), procalcitonin (p = 0.0008), QT interval (p = 0.002), QTc interval (p = 0.04), and QTc dispersion (p = 0.01), and lower values of sodium (p = 0.03), magnesium (p = 0.04), glomerular filtration rate (p < 0.0001), and hemoglobin (p = 0.008) as compared to patients without arrhythmia. By comparing the three subgroups of patients, no significant differences were found. At multivariate analysis, age [odds ratio (OR) = 1.14 (95% CI: 1.07–1.22); p = 0.0004], coronary artery disease [OR = 12.7 (95% CI: 2.38–68.01); p = 0.005], and circulating troponin [OR = 1.05 (95% CI: 1.003–1.10); p = 0.04] represented risk factors independently associated with arrhythmia. All-cause in-hospital mortality was ∼40-fold higher among patients with arrhythmia [OR = 39.66 (95% CI: 5.20–302.51); p = 0.0004].ConclusionArrhythmias are associated with aging, coronary artery disease, subtle myocardial injury, hyperinflammatory status, coagulative unbalance, and prolonged QTc dispersion in patients with COVID-19, and confer a worse in-hospital prognosis. Given its usefulness, routinary use of cardiac telemetry should be encouraged in COVID wards.
