Association between CYP2D6 Genotypes and the Risk of Antidepressant Discontinuation, Dosage Modification and the Occurrence of Maternal Depression during Pregnancy

Abstract: Importance: Polymorphic expression of drug metabolizing enzymes affects the metabolism of antidepressants, and thus can contribute to drug response and/or adverse events. Pregnancy itself can affect CYP2D6 activity with profound variations determined by CYP2D6 genotype. Objective: To investigate the association between CYP2D6 genotype and the risk of antidepressant discontinuation, dosage modification, and the occurrence of maternal CYP2D6, Antidepressants, Depression during pregnancy.Setting: Data from the Org… Show more

“…Substantial pharmacokinetic changes may occur during pregnancy for a number of commonly used antidepressants and mood stabilizers that may alter medication concentrations and thus contribute to drug response and/or adverse events [37]. Also, genetic variation in drug metabolizing enzymes, like cytochrome 2D6 and P450 (CYP2D6 and CYP450), may affect the metabolism of antidepressants and anxiolytics [11, 38].…”

“…Depression is one of the most common complications of the prenatal and postpartum periods [7, 10]. During pregnancy, antidepressants are among the most frequently prescribed medications [11, 12]. The frequency of antidepressant use in pregnant women varies worldwide, ranging from 1.8% to up 10% [1317].…”

“…For example, due to increased maternal metabolism during pregnancy, 21 SSRI drug dosage should be increased to maintain the same prepregnancy effectiveness. 22 However, evidence shows that the majority of women maintain or decrease their SSRI dosage once their pregnancy is diagnosed. 21 Therefore, if there were benefits to SSRI use before pregnancy, this is no longer the case during pregnancy.…”

“…Sample and genotype data were compiled from previous studies (Abduljalil et al, 2010;Gaedigk et al, 2012;Montane Jaime et al, 2013;Tay-Sontheimer et al, 2014;Brown et al, 2016;Bérard et al, 2017;Gaedigk et al, 2019b) as well as ongoing studies (Genomicand Ontogeny-Linked Dose Individualization and cLinical Optimization for Kids (GOLDILOKs), CYP2D6 genotype and neurocognitive dysfunction in methamphetamine users with and without HIV and medication use in pregnant women). Studies into which participants were initially enrolled in were approved by the institutions at which the studies were performed.…”
