Background Multimorbidity is of increasing prevalence and importance. It has been associated with poorer health-related quality of life (HrQoL) especially in the elderly population. Despite substantial multimorbidity at midlife, defined as those between 40-64 years old, there is a paucity of research investigating the impact of multimorbidity in this population. This study aimed to investigate the association between multimorbidity and HrQoL in the middle-aged primary care population in Singapore. Methods A cross-sectional study involving 40-64 year old participants was conducted at a primary care centre in Singapore. Interviewer-administered questionnaires were used to collect data regarding the participants’ sociodemographic characteristics, chronic conditions, and HrQoL, as measured by the EuroQol 5 dimensions 3-levels questionnaire (EQ5D-3L). The associations between multimorbidity and the components of the EQ5D were assessed using multivariable regression analyses. Results The study included 297 participants, aged 40-64 years, of which 124 (42.7%) had multimorbidity. Participants with multimorbidity had lower mean EQ5D Utilities Index (UI) and mean Visual Analogue Scale (VAS) scores (UI = 0.804 ±0.251, VAS = 64.9 ±16.8) compared to those without multimorbidity (UI = 0.871 ±0.198, VAS = 68.5 ±16.0). After adjusting for sociodemographic factors, those with multimorbidity had significantly lower EQ5D UI, (β-coefficient = -0.064 (C.I -0.125, -0.003), p = 0.04), but not significantly lower EQ5D VAS, (β-coefficient = -0.045 (C.I 0.102, 0.012), p = 0.12). Additionally, participants with multimorbidity had higher odds (OR = 2.41, p = 0.01) of reporting problems due to pain/discomfort. Conclusion Multimorbidity was not significantly associated with the overall health state, as measured by the EQ5D VAS, in middle-aged primary care patients. However, it was associated with the EQ5D UI which is a composite measure of five specific domains of HrQoL. Specifically, there was a significant association between multimorbidity and the pain domain. Further studies are required to understand the relationship between multimorbidity and pain to enable physicians to better manage pain and HrQoL in this population.

Describir la calidad de vida relacionada con la salud (CdV) de migrantes venezolanos que residen en las ciudades de Chimbote y Nuevo Chimbote en Perú y evaluar sus factores asociados. Materiales y métodos. Estudio transversal en 212 migrantes venezolanos reclutados por muestreo tipo bola de nieve. La CdV fue evaluada con el European Quality of Life-5 Dimensions en tres niveles (EQ-5D-3L). Los resultados fueron presentados descriptivamente. Para evaluar los factores asociados a CdV se emplearon regresiones lineales y regresiones ajustadas de Poisson. Resultados. El porcentaje de participantes afectados en cada dimensión de CdV fue: movilidad (1,4%), autocuidado (0,5%), actividades cotidianas (2,4%), dolor/malestar (18,4%) y ansiedad/depresión (68,9%). En cuanto a la Escala Visual Análoga (EVA) de CdV, la mediana del puntaje fue 85 (RIC: 70 a 90). Al evaluar los factores asociados, se encontró que ninguno de los factores evaluados se asoció al puntaje en la EVA o a presentar dolor/malestar. Sin embargo, la frecuencia de presentar ansiedad/depresión fue mayor en aquellos con grado de instrucción superior universitario en comparación con aquellos con estudios de secundaria (Razón de prevalencias ajustada 1,28; IC 95%: 1,03 a 1,60). Conclusiones. Los migrantes venezolanos que participaron del estudio reportaron problemas en su CdV, más de dos terceras partes reportaron ansiedad/depresión, y uno de cada seis presentó dolor/malestar. La frecuencia de presentar ansiedad/depresión fue mayor en aquellos con instrucción universitaria. ABSTRACT Objectives.To describe the health-related quality of life (HRQoL) of Venezuelan migrants residing in the cities of Chimbote and Nuevo Chimbote, in Peru, and assess its associated factors. Materials and Methods. Cross-sectional study of 212 Venezuelan migrants recruited by snowball sampling. The QoL was evaluated with the European Quality of Life-5 Dimensions at three levels (EQ-5D-3L). The results were presented descriptively. Linear regressions and Poisson adjusted regressions were used to evaluate the factors associated with QoL. Results. The percentage of participants affected in each QoL dimension was: mobility (1.4%), self-care (0.5%), usual activities (2.4%), pain/discomfort (18.4%), and anxiety/depression (68.9%). As for the QoL Visual Analog Scale (VAS), the median score was 85 (RIC: 70-90). In evaluating the associated factors, it was found that none of the factors was associated with the VAS score or pain/ discomfort. Calidad de vida en pacientes con neuropatía diabética periférica: estudio transversal en Lima, PerúQuality of life among patients with peripheral diabetic neuropathy: a cross-sectional study in Lima, Peru RESUMEN Objetivo: Evaluar la asociación entre la presencia de neuropatía periférica y calidad de vida en pacientes con diabetes mellitus tipo 2. Materiales y métodos: Estudio transversal que enroló pacientes con diabetes mellitus tipo 2, de 18 años a más en un hospital peruano de nivel terciario. La variable resultado fue calidad de vida, evaluada en las esferas física y mental usando el cuestionario 36-item Short Form (SF-36). El diagnóstico de neuropatía periférica fue dado por al menos una de las siguientes: prueba del monofilamento Semmes-Weinstein, prueba del diapasón 128 Hz, úlceras visibles en pie y/o artropatía de Charcot. La asociación de interés se verificó mediante regresión lineal. Resultados: Se incluyeron 330 pacientes, 56,6% mujeres, edad media: 61,3 (±11,5) años. La prevalencia de neuropatía periférica fue de 44,2% (IC95%: 38,8%-49,6%). De acuerdo a la esfera física del SF-36, la calidad de vida media fue de 46,8 (±6,3) puntos; mientras, la media de la calidad de vida en la esfera mental fue de 39,5 (±8,2) puntos. En modelo multivariable, la neuropatía periférica estuvo asociada a una reducción de dos puntos (β = -2,06; IC95%: -3,52; -0,60) en la calidad de vida en la esfera física, pero no afectó la calidad de vida en la esfera mental (β = 0,03; IC95%: -1,79; 1,85). Conclusiones: Existe asociación entre la presencia de neuropatía diabética periférica y calidad de vida en la esfera física, pero no en la esfera mental. ABSTRACTObjective: To assess the association between the presence of peripheral neuropathy and life quality among patients with type 2 diabetes mellitus. Materials and methods: A cross-sectional study enrolling patients aged ≥18 years with diagnosis of type 2 diabetes mellitus in a tertiary-level hospital. The outcome was quality of life, assessed in the physical and mental spheres of the 36-item Short Form (SF-36). The diagnosis of peripheral neuropathy was given by the positivity of one of the following tests: Semmes-Weinstein monofilament test, 128 Hz tuning fork test, presence of visible foot ulcers and/or Charcot arthropathy. The linear regression model was used to verify the association of interest. Results: A total of 330 patients were enrolled, 56.6% female, mean age 61.3 years (±11.5). The prevalence of peripheral neuropathy was 44.2% (95%CI: 38.8%-49.6%).
