In individuals with diabetes mellitus (DM) and hypertension, renin-angiotensin system (RAS) inhibitors are advised for the therapy of albuminuria, although there is limited agreement on other treatments. Calcium channel blockers (CCB) are often used for hypertension patients, their effectiveness in those with albuminuria remains uncertain. Patients with diabetes, hypertension and nephropathy were evaluated for their response to CCB and inhibitors of the RAS to reduce albuminuria. We measured albuminuria before and after the intervention and compared CCB to RAS inhibitors by looking through records on MEDLINE, Embase and CENTRAL. Randomized controlled adult trials were identified using abstract screening were performed separately. 20 papers were selected from 800 records using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA). We evaluated data from 2,114 participants in the experiment who had the equivalent of pre-hypertension and diabetic mellitus of ≥31 days/mg of urine albumin excretion. The standardized mean difference in reducing albuminuria was 0.443. When analyzing the treatment impact on blood pressure, the research showed no significant difference between RAS inhibitors and CCB in terms of indications of renal function. RAS inhibitors could be superior to CCB in reducing albuminuria in hypertensive and diabetic nephropathy.

Objectives: Health information systems (HIS) commonly contain patient addresses that provide valuable data for geocoding and spatial analysis, enabling more comprehensive descriptions of individual patients in biomedical studies. Despite the widespread adoption of HIS, no systematic review has examined the extent to which spatial analysis is used in characterizing patient phenotypes. Materials and Methods: We systematically evaluated English-language peer-reviewed articles from PubMed/MEDLINE, Scopus, Web of Science, and Google Scholar databases from inception to August 20, 2023, without imposing constraints on time, geography, or specific health domains. Results: Only 62 articles met the eligibility criteria. These articles utilized diverse spatial methods, with a predominant focus on clustering techniques, while spatiotemporal analysis (frequentist and Bayesian) and modeling were relatively underexplored. Geographically, the use was limited, involving only nine countries, with over 80% of studies conducted in the United States. Moreover, a noteworthy surge (82.3%) in publications was observed post-2017. The publications investigated various clinical areas, including infectious disease, endocrinology, and cardiology, using phenotypes defined over a range of data domains, such as demographics, diagnoses, and visit. The primary health outcomes investigated were asthma, hypertension, and diabetes. Notably, patient phenotypes involving genomics, imaging, and notes were rarely utilized. Discussion and Conclusion: This review underscores the growing interest in spatial analysis of HIS-derived data and highlights knowledge gaps in clinical health, phenotype domains, geospatial distribution, and spatial methodologies. Additionally, this review proposes guidelines for harnessing the potential of spatial analysis to enhance the context of individual patients for future biomedical research.

Background This study aims to examine the effect of time‐variant perceived neighborhood social cohesion, perceived neighborhood physical disorder, and local crime on cardiovascular disease (CVD) incidence from 2006 through 2016. Methods and Results We obtained data from the Health & Retirement Study. Respondents aged ≥50 years and with no recorded history of CVD until 2006 (N=8826) were included and followed for 10 years. Cox proportional hazards models were estimated with CVD incidence as an outcome variable and time‐variant social environment factors (perceived neighborhood social cohesion, perceived neighborhood physical disorder, and local crime) as exposures, after controlling for sociodemographic factors and CVD‐related risk/protective factors. Our results showed that perceived neighborhood social cohesion was associated with CVD among Black respondents, but not Hispanic and White respondents. Perceived neighborhood physical disorder and local crime rates were not associated with CVD incidence across all racial and ethnic groups. Conclusions The results demonstrate that perceptions of favorable social environments need to be considered to reduce CVD risk among Black adults. Further research is needed to identify different pathways through which living in favorable social environments benefits cardiovascular health by racial and ethnic groups.
