Aims To compare the value of the proximal flow convergence method and the jet area method for the determination of the severity of tricuspid regurgitation. Methods and ResultsThe proximal isovelocity surface area radius and the jet area/length were measured in 71 consecutive patients with angiographically graded (grade 0/I-III) tricuspid regurgitation. Rank correlation coefficients with the angiographic grade were 0·71 (P<0·001) for the proximal isovelocity surface area radius (aliasing border of 28 cm . s 1 ), 0·66 (P<0·001) for the jet area, and 0·63 (P<0·001) for the jet length. The proximal isovelocity surface area radius was significantly correlated with the jet area/length (correlation coefficients 0·82/0·77, P<0·001). Correct differentiation between mild to moderate (grade I-II) and severe (grade III) tricuspid regurgitation was achieved in 62 of 71 patients (87%) by means of the proximal isovelocity surface area radius, in 61 of 71 (86%) by the jet area, and in 62 of 71 (87%) by the jet length. Grade III tricuspid regurgitation was not identified in five of 21 patients (24%) by means of the proximal isovelocity surface area radius, in six of 21 (29%) by the jet area, and in seven of 21 (33%) by the jet length. ConclusionThe flow convergence method and the jet area method are of similar value for the determination of the severity of tricuspid regurgitation. Both methods differentiated mild to moderate from severe tricuspid regurgitation in most patients. However, underestimation of severe tricuspid regurgitation in 20-30% of the cases represents a serious limitation of both methods.

This finding may associate with patent foramen ovale that it is relatively common in the community. No study was found investigating the association between tricuspid regurgitation and patent foramen ovale. The aim of this study was to evaluate the association between tricuspid regurgitation and patent foramen ovale in healthy young subjects. Subjects who had evidence of cardiovascular or other systemic disease were excluded from the study. Detection of tricuspid regurgitation was performed with color flow Doppler. The tricuspid regurgitation jet length and area were measured using color Doppler images. The tricuspid regurgitation jet peak velocity and duration were measured with continuous wave Doppler modality. Study subjects were divided into two groups according to whether patent foramen ovale was present [PFO (+) group and PFO (-) group]. R Re es su ul lt ts s: : There were 23 subjects in the PFO (+) group and 65 subjects in the PFO (-) group. Tricuspid regurgitation flow was pansystolic in 70% of subjects in the PFO (+) group and 8% of subjects in the PFO (-) group (p<0.001). Sensitivity and specificity pansystolic tricuspid regurgitation for detecting patent foramen ovale were 70% and 92% respectively. The evaluation and management of tricuspid regurgitation (TR) are often challenging. Significant TR is an independent predictor of reduced event-free and overall survival. Therefore, an evidence-based approach to the diagnosis and treatment of TR is of critical importance. TR can be classified into two basic categories: primary and secondary TR. The former refers to conditions in which the primary pathophysiologic process affects the valve itself, whereas the latter is much more common and occurs due to tricuspid annular dilatation, right heart failure, and/or pulmonary hypertension. Two- and three-dimensional echocardiography allow for a comprehensive assessment of TR severity and mechanisms. In patients with fixed pulmonary hypertension and right ventricular dysfunction, medical management of TR is generally preferable. In patients undergoing mitral valve surgery, tricuspid annular dilatation should trigger prophylactic tricuspid valve repair, regardless of the degree of TR. Future efforts in TR management will include development of percutaneous repair procedures.
