Assessment of role of perioperative melatonin in prevention and treatment of postoperative delirium after hip arthroplasty under spinal anesthesia in the elderly

Abstract: Context:Little is known about the relationship between sedative drugs used preoperatively and postoperative delirium. Melatonin is a drug used to sedate patients preoperatively and is hypothesized by recent works to have a curative effect on postoperative delirium.Aims:The incidence of postoperative delirium will be tested if affected by three different sedative drugs including melatonin.Settings and Design:Controlled randomized doubleblind study.Patients and Methods:Three-hundred patients aged>65 years schedu… Show more

“…These results indicate that, for vulnerable elderly patients with hip fracture, melatonin is not effective in preventing postoperative delir ium. Recently, 2 other randomized controlled trials showed substantial decreases in the inci dence of delirium among patients receiving mela tonin (33% v. 9% 24 and 31% v. 12% 23 ). The earlier of these 2 studies, 24 which involved patients undergoing elective surgery, had some methodologic drawbacks due to a lack of clarity about the randomization procedure and blinding methods.…”

“…[17][18][19][20][21][22] Although proof of a causal relation is still lacking, inpatients might nevertheless bene fit from melatonin supplementation therapy through postoperative maintenance or restoration of their sleep-wake cycle. [23][24][25] Although melato…”

“…В исследовании, про-веденном с участием пожилых пациентов стациона-ра, мелатонин существенно снижал риск возникно-вения делирия (c 31 до 12%) [71], но не было уста-новлено влияния мелатонина на тяжесть симптомов делирия или длительность госпитализации. В дру-гой работе [108] было отмечено снижение частоты делирия при использовании мелатонина у хирурги-ческих больных.…”

“…9 Two clinical trials have shown that exogenous low-dose melatonin administered nightly to elderly medical or elective surgical patients was successful in decreasing the incidence of delirium. [10][11][12] Ramelteon demonstrated a high affinity for melatonin receptors, with a longer half-life and better efficacy in the treatment of insomnia than melatonin. 13 It has also been used to prevent delirium in a randomized controlled trial involving older patients with serious medical problems.…”
