Introduction: Health Related Quality of Life (HRQoL) is an important outcome measure to investigate and monitor patients with chronic diseases. In order to achieve such goal, it is essential to choose an appropriate tool to evaluate the peculiarities of each population. Instruments to investigate HRQoL, NHP and SF-36 are alike in several aspects and are considered by the scientific community as replaceable each other. However, there are doubts regarding the agreement of their social domains. Objective: The present study aim to assess whether the social domains of the NHP and SF-36 correspond when applied in a population with chronic disease. Methods: The present research was made by applying the two instruments to a population with chronic disease. The social domains agreement was evaluated by Pearson correlation, ROC curve, AUC, Youdex Index, and Bland-Altman plot. Results: It was achieve a weak correlation between the two social domais (r = 0.3), confirmed by ROC curve with small AUC (0.416) and Youdex Index around 0.0. The Bland-Altman plot reaffirmed the no agreement between the two subscale by achieving mean difference equal to 29.37 (± 38.51), which made the concordance Interval vary from 106.39 to -47.65. Conclusion: it was concluded that social domains of NHP and SF-36 do not agree to each other. However, reasons for that are not explained and need to be investigated in futures studies. The current research was approved by the Research Ethics Committee of the Bahiana School of Medicine and Public Health under the protocol CAAE 516.42315.5.0000.5544.

Multivariate outcomes are common in pragmatic cluster randomized trials. While sample size calculation procedures for multivariate outcomes exist under parallel assignment, none have been developed for a stepped wedge design. In this article, we present computationally efficient power and sample size procedures for stepped wedge cluster randomized trials (SW‐CRTs) with multivariate outcomes that differentiate the within‐period and between‐period intracluster correlation coefficients (ICCs). Under a multivariate linear mixed model, we derive the joint distribution of the intervention test statistics which can be used for determining power under different hypotheses and provide an example using the commonly utilized intersection‐union test for co‐primary outcomes. Simplifications under a common treatment effect and common ICCs across endpoints and an extension to closed‐cohort designs are also provided. Finally, under the common ICC across endpoints assumption, we formally prove that the multivariate linear mixed model leads to a more efficient treatment effect estimator compared to the univariate linear mixed model, providing a rigorous justification on the use of the former with multivariate outcomes. We illustrate application of the proposed methods using data from an existing SW‐CRT and present extensive simulations to validate the methods.

Health related quality of life in 2002 and 2015 in patients undergoing hemodialysis: a single center study

Health related quality of life (HRQOL) has become a recognized outcome in studies of the treatment of patients undergoing hemodialysis (HD). The aim of this study was to assess changes in HRQOL reported by two different samples of patients undergoing HD in the same dialysis center in 2002 and 2015. Two samples of patients from the same Danish dialysis center completed the HRQOL questionnaire Kidney Disease Quality of Life version 1.3 in 2002 (n = 71) and 2015 (n = 81), respectively. The associations between KDQOL summery scores and the time frame were analyzed in multivariable regression analyses. The 2015 sample of patients were older compared to the 2002 sample (66 ± 13 vs. 59 ± 16 years, p = 0.003, respectively), they comprised more obese patients (20.6 vs. 3.2%, p = 0.002) and more patients with hypoalbuminemia (66.7 vs. 42.9%, p = 0.006). After confounder adjustment the time period 2015 versus 2002 was not associated with changes in the generic summery scales Physical Component Scale (β 3.9, 95% CI <-0.1; 7.9, p = 0.051) and Mental Component Scale (β -2.5, 95% CI -7.6; 2.5, p = 0.322) or the disease specific summery scale Kidney Disease Component Scale (β 1.0, 95% CI -5.1; 7.1, p = 0.746). This study indicated an unchanged HRQOL when recent data were compared with data collected 13 years earlier in patients undergoing HD from a Danish dialysis center. We suggest that the included dialysis center monitor HRQOL and pay attention on interventions that may be associated with improvements of HRQOL and reduce the elevated frequency of patients with obesity or hypoalbuminemia.
