With the tremendous advances in cardiac pacing during the past four decades, cardiac pacemaker implantation is now a common clinical procedure. In recent years, the indications for permanent pacemakers have expanded. This increase in reasons for pacing and shift in mode of pacing have been caused by the evolution of pacemaker therapy from a life-saving measure (mortality), to one aimed at improving healthrelated quality of life (HRQoL). Until now the efficacy of pacing therapy has predominantly been measured using bobjectiveQ criteria. However, in recent years the importance of HRQoL as an outcome measure has increasingly been recognized as patients prefer quality over quantity of life.In this review we describe the development and testing of Aquarel, a new developed HRQoL questionnaire for pacemaker patients, composed of a generic core module with disease specific add-ons. Current and future research to improve the Aquarel questionnaire is also described. D 2005 Elsevier Ireland Ltd. All rights reserved.Keywords: Cardiac pacing; Quality of life; Aquarel In the Netherlands there is an increased implantation rate of cardiac pacemakers in patients of all ages over the past decade (Fig. 1). This increase in the number of implanted pacemaker-units and leads combined with a rising population age contribute to an increase in hospitalisation, care and follow-up. Until now the efficacy of pacing therapy has predominantly been measured using clinical outcomes such as improvement of prognosis, relief of symptoms, complications, technical failures and recalls. However, to evaluate the full spectrum of outcome of pacemaker therapy patient reported outcomes as well as clinical outcomes should be measured. In recent years the importance of health-related quality of life (HRQoL) as an outcome measure has increasingly been recognized [1,2]. This has particular relevance in cardiac pacing because the goal of therapy for most patients with chronic disease is improvement in function, not cure [3]. These measures should be incorporated as one of the primary measures of outcome in the evaluation of new therapies in chronic diseases including cardiac pacing. Doing so, clinicians, patients, policymakers, health care providers and HRQoL researchers gain experience with these measures.To measure HRQoL, appropriate instruments should be used and a large number of questionnaires has been developed in the past two decades. However, in the field of cardiac pacing until recently several instruments were used to measure HRQoL that were not properly validated and applied [4][5][6][7][8][9][10][11][12][13][14][15][16][17]. Consequently these HRQoL data should be interpreted cautiously. HRQoL is typically measured either in a generic or a specific way. Generic means that an instrument gives information on several dimensions and is useful for a variety of illnesses, diseases and different (patient-) populations, allowing for comparison between these (patient-) populations. Abstract. Various aspects of the way of life in 20 adult patients (mean age: 25.7±6.0) with Turner syndrome were studied for their quality of life (QOL). The study found that many more Turner women went on to university (P<0.01) than the general population, whereas GH-deficient women did not. The employment status of both Turner and GH-deficient women does not differ from that of the general population. Only 7 of the 12 Turner and 5 of the 15 GH-deficient employees are satisfied with their present jobs, but 10 of the former and 7 of the latter women think that their jobs are worthwhile. The total annual income of 3 of 19 Turner women exceeded 3 million yen, but not that of any of 10 GHdeficient women who answered the question on their income. Half of the Turner and GH-deficient women complained of shoulder stiffness and one fourth of the Turner and one fifth of the GH-deficient women have a sense of despair and irritability. Seven Turner and 9 GH-deficient women have anxiety about their body, and 6 of the former and 8 of the latter have anxiety about marriage. Only 4 Turner and 3 GH-deficient women are married. Three of the former marriages were arranged by their parents, but all 3 GH-deficient women got married after falling in love. These marriage rates are significantly lower (P<0.005) than that of the age-matched general population.Most of the unmarried women in both groups live with their parents, and half of the Turner and one third of the GH-deficient women have difficulty in getting clothes and shoes to fit them. Otherwise, they have no particular problem in their daily life. In conclusion, Turner women are well educated and work as normal women, but they, as well as GH-deficient women, appear to be anxious about their body and marriage.

Резюме. В настоящее время при многих хронических заболеваниях все большее значение придается исследованию качества жизни (КЖ). Сахарный диабет (СД) в силу сложного режима контроля и лече-ния, а также многочисленных соматических проявлений (острых и хронических осложнений) оказывает ВведениеПо данным ВОЗ, распространенность сахарного ди-абета (СД) составляет от 2 до 6 %. При сахарном диабете поражаются артерии всех органов и калибров с развитием микроангиопатии в 100 % случаев и макроангиопатии -у 70 % больных [1]. При диабетической ангиопатии у 30 % больных наблюдаются гнойно-некротические осложне-ния. Через 15-20 лет после манифестации СД 2-го типа у 50 % больных отмечается окклюзия магистральных артерий, приводящая к развитию гангрены нижних ко-нечностей в 20-40 раз чаще, чем у больных без наруше-ний углеводного обмена [19]. В структуре всех ампутаций нижних конечностей нетравматического характера боль-ные СД составляют 50-70 %.
