Background The use of low level laser therapy (LLLT) to reduce injection pain associated with dental local anesthesia is reported in a limited number of studies in adults, but research on the effects of LLLT in children is needed. Aim This study aimed to evaluate the effects of topical anesthesia + LLLT on injection pain, anesthesia efficacy, and duration in local anesthesia of children who are undergoing pulpotomy treatment. Design The study was conducted as a randomized, controlled‐crossover, double‐blind clinical trial with 60 children aged 6–9 years. Before local infiltration anesthesia was administered, only topical anesthesia was applied in one side (control group/CG), and topical anesthesia plus LLLT (a diode laser: 810 nm; continuous mode; 0.3W; 20 s; 69 J/cm2) was applied in the contralateral side (LG) as pre‐anesthesia. The injection pain and anesthesia efficacy were evaluated subjectively and objectively using the Wong‐Baker Faces Pain Rating Scale (PRS) and the Face, Legs, Activity, Cry, Consolability (FLACC) scale respectively. Data were analyzed for statistical significance (p < .05). Results The “no pain” and “severe pain” rates in the PRS were 41.7% and 3.3% for the LG and 21.7% and 11.7% for the CG, respectively, during injection. Similarly, in the FLACC data, the number of “no pain” responses was higher for the LG than the CG (40%, 33.3%) and no “severe pain” rate was observed in both groups. The only statistically significant difference found for the PRS was p < .05. The median pain score was “0” for the LG and the CG in the FLACC data for the evaluation of anesthesia efficacy, and there was no statistically significant difference between the groups in terms of pain and anesthesia duration (p > .05). Also, most of the children preferred injection with topical anesthesia + LLLT (66.7%). Conclusions It has been determined that the application of topical anesthesia + LLLT with an 810‐nm diode laser before local infiltration anesthesia reduced injection pain and did not have an effect on anesthesia efficacy and duration in children.

Introdução: Os lasers são classificados em lasers de alta intensidade e de baixa intensidade. Sua aplicabilidade pode ser utilizada em vários procedimentos clínicos na Odontopediatria, como exemplo na cirurgia de freio labial e lingual, em preparos cavitários e na biomodulação de lesões orais. Objetivo: Verificar na literatura disponível os tipos de lasers utilizados em Odontologia e seu uso na prática Odontopediátrica. Metodologia: Trata-se de uma revisão integrativa da literatura, onde foi feito um levantamento bibliográfico através das bases de dados ScieLO e PubMed entre os anos de 2015 e 2020. Resultados: Foram selecionados, um total de 15 artigos dos quais 6 estudos foram Ensaios Clínicos Randomizados, 4 artigos de Revisão Sistemática e 5 Revisões de Literatura. Das pesquisas feitas com o laser de alta potência, os resultados encontrados foram favoráveis no tratamento da frenuloplastia, quando comparado à técnica pelo bisturi, assim como mostrou-se útil na área da Dentística utilizando-o no preparo de cavidades e de superfícies dentárias para aplicação de selantes em relação ao uso da broca. Já o laser de baixa intensidade demonstrou ser uma alternativa excelente para uso pós-exodontias como também como terapia de fotobiomodulação em pacientes que possuem lesões orais. Também foi visto resultados positivos, no uso do laser de baixa potência na cicatrização e regeneração óssea após ser feita a expansão rápida da maxila. Conclusão: Tanto os lasers de alta e de baixa potência são boas opções para os Cirurgiões-Dentistas, principalmente os Odontopediatras, pois promove uma boa recuperação, um maior conforto e aceitabilidade pelo público infantil.

The success of using 2% lidocaine in pain removal during extraction of mandibular premolars: a prospective clinical study

Background: The purpose of this study was to evaluate the anesthetic effectiveness of a buccal infiltration technique combined with local massage (using 2% lidocaine) in the extraction of mandibular premolars to be utilized as an alternative to the conventional inferior alveolar nerve block. Methods: Patients eligible included any subject with a clinical indication for tooth extraction of the mandibular 1st or 2nd premolars. All patients were anesthetized buccally by local infiltration technique followed by an external pressure applied for 1 min directly over the injection area. In each case, another local injection was given lingually. All operations were started at approximately 5 min after the buccal injection. The collected data included age, gender, pain perception and its intensity during treatment at three checkpoints, apical tenderness, and the type of extraction. Any associated complications or difficulties were also recorded. Then the results were analyzed and interpreted using appropriate statistical tests. The significance level was set at P ≤ 0.05. Results: A total of 247 cases (1st premolar, n = 119; 2nd premolar, n = 128), predominantly male, were included. In 95% of study sample, the patients were satisfied with the dental extraction without any pain. However, in 5% of cases, pain was reported at the stage of tooth removal. Apical tenderness was found to be present in 11% of the total cases. Three teeth required surgical removal. Upon analysis, no significant differences in the success rates were detected between the 2 premolar groups or amongst the various age groups. Minor and transient side effects were reported in this study. Conclusion: The technique is simple and effective as well. It might be considered as an alternative anesthetic injection to the inferior alveolar nerve block for dental extraction of the mandibular premolars.
