Assessment of noninvasive predictors of bladder detrusor underactivity in BPH/LUTs patients

Abstract: Objective: Compare detrusor muscle of normal and patients with infravesical obstruction, quantifying the collagen and elastic system fibers. Materials and Methods: We studied samples taken from bladders of 10 patients whose ages ranged from 45 to 75 years (mean = 60 years), who underwent transvesical prostatectomy for treatment of BPH. Control material was composed of 10 vesical specimens, removed during autopsies performed in cadavers of accident victims, with ages between 18 and 35 years (mean = 26 years). R… Show more

“…In line with the abovementioned studies, in our study Qmax presented an accuracy on ROC analysis of 0.76 (data not shown). The role of PV as a predictor of DUA has been as well evaluated by Luo et al 8 with an estimated accuracy of 0.74 8 . According to our results, the predictive accuracy was 0.61 on ROC analysis (data not shown).…”

“…According to their results, Qmax presented a 0.76 accuracy for the prediction of DU defined as BCI < 100 mm Hg 7 . As well, Luo et al 8 evaluated retrospectively noninvasive predictors of DUA in a cohort of 704 patients with LUTS undergoing urodynamic studies. In their experience, Qmax reached an accuracy of 0.74 on ROC analysis for the prediction of DUA defined as maximum detrusor pressure < 50 mmH20.…”

“…Further mechanistic and epidemiologic classification of the factors which result in UAB need to be identified to improve the detection of DU and guide treatment. Existing noninvasive assessments of lower urinary tract function include PVR, uroflowmetry, detrusor wall thickness, near‐infrared spectroscopy, and penile compression release . While suggestive of BOO, there is a very limited assessment of these measures in a prospective approach to determine their ability to discriminate DU from BOO.…”

Invasive urodynamic pressure-flow evaluation of voiding function remains the reference standard test used to diagnose BOO and DU.…”
