Objectives: Malocclusion was and remains one of the most common problems which affects the psyche and social status of the individual, so the estimation of the malocclusion severity and needs a percentage of orthodontic treatment of Iraqi patients is the aim of this study.Method: A randomly selected 150 pairs of study models (48 male and 102 female) were involved in this study for patients attending an orthodontic clinic at College of Dentistry/ University of Baghdad seeking for treatment. The DAI scores were collected according to WHO guidelines directly from the study model with a digital caliper, score was calculated using the regression equation of 10 occlusal traits. The dental casts were classified into four groups to determine the treatment needs. SPSS software version 25 was used to analyze the results.Results: 8.7% of orthodontic treated patients were with normal or mild malocclusion (DAI≤25) and did not need treatment, while 56% of them were handicapped and needed mandatory treatment. In between them, patients with definite and severe malocclusion were 18.7% and 16.6% respectively. Molar deviation (72.6%) represents a high prevalence rate among occlusal traits, while negative overjet (7.3%) is the least one.Conclusions: Not all patients who are treated in an orthodontic clinic are really in need, and it must be the implementation of treatment need index to determine the treatment priority for patients.

Objetivo: Revisar sistematicamente as evidências existentes sobre a prevalência da necessidade de tratamento ortodôntico em jovens de 12 a 15 anos de idade.Métodos: Realizou-se uma busca eletrônica nas bases de dados Portal Regional da BVS, PubMed, Scielo, Cochrane, Web of Science, Embase e Scopus utilizando descritores e termos utilizados nas publicações. Artigos duplicados foram removidos. Após a aplicação dos critérios de exclusão e inclusão, a qualidade dos artigos foi verificada e a extração de dados realizada.Resultados: Dos 2255 estudos encontrados, 1730 foram removidos por duplicidade, 487 artigos foram eliminados segundo os critérios de exclusão e 11 estudos foram eleitos de acordo com os critérios de inclusão.Conclusão: A prevalência da necessidade de tratamento ortodôntico foi considerada alta em diferentes regiões do mundo, principalmente na população brasileira, japonesa e africana. Recomenda-se fortemente a intensificação da educação em saúde bucal em adolescentes, com o propósito de conscientizá-los sobre a importância da prevenção das oclusopatias e a busca por assistência periódica para evitar a necessidade de tratamentos dispendiosos no futuro. Destaca-se também, que é essencial a avaliação criteriosa da qualidade dos estudos epidemiológicos para compreender a real necessidade de tratamento da população-alvo e a implantação de políticas públicas em saúde eficazes.

Aim: To determine the mean Dental Aesthetic Index (DAI) malocclusion score among patients with Antero-posterior basal bone discrepancy (APBBD) and to assess the treatment need of the patients with horizontal jaw deformities as calculated by DAI guidelines. Methodology: The descriptive cross sectional study was conducted in the Orthodontic department, University College of Dentistry, The University of Lahore. Lateral cephalograms of the patients were analyzed for anteroposterior basal bone discrepancy by measuring angle ANB. Two hundred and thirty one subjects were included in the study having ANB >4° or <1° through non probability consecutive sampling technique. Study casts were obtained and malocclusion parameters for Dental Aesthetic Index (DAI) were calculated. Results: The mean Dental Aesthetic Index (DAI) score in APBBD was 37.10 ± 7.62, in skeletal class II (37.14 ± 7.90) and in skeletal class III (36.98 ± 6.68), (p-value 0.75). Moderate positive correlation existed between DAI score and ANB in skeletal class II subjects (R 0.49, p <0.001). Conclusion: The mean DAI score in APBBD showed mandatory treatment need in patients with Antero-posterior basal bone discrepancy. Moderately positive correlation existed between ANB and DAI score in skeletal class II patients. Keywords: Malocclusion, Dental Aesthetic Index, Jaw disharmony
