Objective African-Americans have a strikingly low prevalence of ideal cardiovascular health metrics of the American Heart Association’s Life’s Simple 7 (LS7). This study was conducted to assess the impact of a community-based cardio-vascular disease prevention intervention on the knowledge and achievement of cardiovascular health metrics among a marginalized African-American community. Methods Adult congregants (n = 37, 70 % women) from three African-American churches in Rochester, MN, participated in the Fostering African-American Improvement in Total Health (FAITH!) program, a theory-based, culturally-tailored, 16-week education series incorporating the American Heart Association’s LS7 framework. Feasibility testing included assessments of participant recruitment, program attendance, and retention. We classified participants according to definitions of ideal, intermediate, and poor cardiovascular health based on cardiac risk factors and health behaviors and calculated an LS7 score (range 0 to 14) at baseline and post-intervention. Knowledge of cardiac risk factors was assessed by questionnaire. Main outcome measures were changes in cardiovascular health knowledge and cardiovascular health components related to LS7 from baseline to post-intervention. Psychosocial measures included socioeconomic status, outlook on life, self-reported health, self-efficacy, and family support. Results Thirty-six out of 37 recruited participants completed the entire program including health assessments. Participants attended 63.5 % of the education series and attendance at each session was, on average, 62 % of those enrolled. There was a statistically significant improvement in cardiovascular health knowledge (p < 0.02). A higher percentage of participants meeting either ideal or intermediate LS7 score categories and a lower percentage within the poor category were observed. Higher LS7 scores correlated with higher psychosocial measures ratings. Conclusions Although small, our study suggests that the FAITH! program is a feasible, community intervention promoting ideal cardiovascular health that has the potential to improve cardiovascular health literacy and LS7 among African-Americans.

Background Community-level factors have been clearly linked to health outcomes, but are challenging to incorporate into medical practice. Increasing use of electronic health records (EHRs) makes patient-level data available for researchers in a systematic and accessible way, but these data remain siloed from community-level data relevant to health. Purpose This study sought to link community and EHR data from an older female patient cohort participating in an ongoing intervention at the Ohio State University Wexner Medical Center to associate community-level data with patient-level cardiovascular health (CVH) as well as to assess the utility of this EHR integration methodology. Materials and Methods CVH was characterized among patients using available EHR data collected May through July of 2013. EHR data for 153 patients were linked to United States census-tract level data to explore feasibility and insights gained from combining these disparate data sources. Analyses were conducted in 2014. Results Using the linked data, weekly per capita expenditure on fruits and vegetables was found to be significantly associated with CVH at the p<0.05 level and three other community-level attributes (median income, average household size, and unemployment rate) were associated with CVH at the p<0.10 level. Conclusions This work paves the way for future integration of community and EHR-based data into patient care as a novel methodology to gain insight into multi-level factors that affect CVH and other health outcomes. Further, our findings demonstrate the specific architectural and functional challenges associated with integrating decision support technologies and geographic information to support tailored and patient-centered decision making therein.

Reducing Stroke Risk Among Young Adult African Americans: A Feasibility Study

African Americans are disproportionately affected by stroke and its modifiable risk factors, and strategies are needed to help African Americans adopt healthy lifestyles early in life. The purpose of this study was to assess the feasibility of the Stroke Counseling for Risk Reduction (SCORRE) intervention, a tailored, theory-based intervention consisting of a motivational video of young African American stroke survivors, the American Heart Association's Life's Simple 7® (LS7) risk assessment and education tool, and a 6-week behavioral risk reduction diary. Feasibility and changes in theoretical variables were evaluated over 6 weeks. In four months, 30 participants were enrolled and randomized and 29 (97%) completed the study. Participants averaged 23 years old, most were female university students, and they had an average of 2.1 out of 7 risk factors for stroke. Only 7% had missing items on questionnaires, and 17% had missing daily diary entries. Although statistical significance of differences was not assessed, both intervention and control groups had improvements in knowledge scores following the initial session, and larger improvements in accuracy of perceived stroke risk and in health behaviors (physical activity and diet quality) were observed among participants receiving SCORRE. An exit survey indicated the majority of participants receiving SCORRE were motivated to achieve better health; enjoyed the video, LS7 and diary; and felt the program length was appropriate. Results suggest that SCORRE is feasible and acceptable, and with additional refinement and further testing has potential for reducing stroke risk among African Americans. © 2016 Wiley Periodicals, Inc.
