Assessment of instability in type B pelvic ring fractures

Abstract: Pelvic ring fractures have increased in incidence and operative fixation over the past several decades. These are dynamic injuries but decisions on operative management are still often made on the basis of static imaging. Expert opinion varies greatly on which injuries require fixation and how much fixation. Examination under anaesthesia has been shown to guide management of pelvic injuries by more accurately assessing levels of instability.

“…7,8 Owing to the recoil capacity of the pelvic ring, x-ray or computed tomography scan might not reflect the potential displacement or instability of the injured pelvic ring. 9,10 Compared to the anterior structure, the posterior part is more important for hemorrhage control and load transmission from the lower limbs to the spine. The sacrum is often fractured because of its thin cortex, causing neurological deficits constantly.…”

“…Management of a pelvic ring injury poses significant challenges for orthopaedic surgeons [ 12 ]. An initial X-ray or CT scan in the emergency department might not reflect the potential displacement or instability of the injured pelvic ring due to the recoil capacity of the pelvis [ 13 ]. The posterior part of the pelvic ring is the key structure in terms of hemorrhage control and load transmission from the lower limbs to the lumbar spine.…”

“…Они возникают в большинстве случаев в результате высокоэнергетических травм, главным образом вследствие дорожнотранспортного происшествия (64,37 %), падения со значительной высоты (25,32 %) либо вследствие спортивных травм (около 2 %) [15,16]. Такие переломы зачастую сопровождаются сопутствующими опасными для жизни травмами [17,18]. Повреждения тазового кольца типа B представляют собой открытые и боковые компрессионные переломы.…”
