Assessment of Dentoalveolar Changes Following Leveling and Alignment of Severely Crowded Upper Anterior Teeth Using Self-Ligating Brackets Alone or With Flapless Piezocision Compared to Traditional Brackets: A Randomized Controlled Clinical Trial

Abstract: IntroductionDental crowding is one of the most common types of malocclusions. It can be treated with or without extraction, depending on the severity of the crowding. Extraction-based orthodontic treatments are the preferred treatment option in cases of severe crowding, but they take longer than non-extraction cases.

“…Micro-etched bonding pads, designed to increase binding strength by 15%-30% over varied bases, are among the new features. Another is a larger clip, which increases wire retention strength while preventing clip deformity [ 34 - 37 ].…”

“…Many attempts have been made clinically and in vivo to find different methods that achieve faster results and reduce the duration of orthodontic treatment, including surgical methods such as traditional corticotomy [ 6 ], flapless corticotomy [ 7 ], laser-assisted flapless corticotomy [ 8 ], piezocision [ 9 ] and corticision [ 10 ] that depend mainly on inducing the regional acceleratory phenomenon (RAP). Or non-surgical methods such as low-level laser therapy (LLLT)[ 11 ], biomechanical appliance modification [ 12 ], and pulsed electromagnetic fields [ 13 ]. Those non-invasive techniques to accelerate orthodontic tooth movement have achieved promising results as surgical methods with greater patient acceptance [ 14 ].…”

“…The patient was instructed post-operative regimen, which included taking two tablets of antibiotic per day for one week after corticotomy, applying ice packs for the first 12 hours after corticotomy, eating soft food in the first three days, using a mouthwash during the first few days, and avoiding smoking during the first week. Instead of taking non-steroid anti-inflammatory drugs, which were not permitted to avoid overlapping with the RAP effect, only 500 mg acetaminophen (Panadol) was allowed to be taken when necessary [ 31 , 32 ].…”
