Assessment of Competence in EVAR Stent Graft Sizing and Selection

Abstract: This study presents a standardised and objective assessment tool of competence in vessel analysis and stent graft selection for endovascular aortic repair. This was supported by strong validity evidence with good internal consistency and discriminatory ability. The tool may be used to facilitate training and certification of future endovascular specialists.

“…15 The test, and the test case DICOM, is available as supplemental material from the European Journal of Vascular and Endovascular Surgery (EJVES). 13 The case allowed the full use of all functionalities of the imaging software, including application of semi-automated centrelines and stretched view of the anatomy. 5 Individual test scores were not part of formative feedback given during the course and failing participants was not an objective of this explorative study.…”

“…A mastery learning approach is warranted and previously validated assessment tools in both EVAR sizing and procedural competence have been published. 6,7,13 Key opinion leaders from 34 countries across Europe have recently gained consensus on the curricular content for simulationbased training in vascular surgery in which EVAR and "planning and sizing" were ranked as the sixth most important skill to learn during vascular surgical training. 14 How vascular trainees can achieve sufficient skills in EVAR sizing and graft selection following a predefined curriculum and workshop format is sparsely explored although essential for successful EVAR implantation.…”

“…9 The case had previously been used in a recent publication of EVAR sizing and stent graft selection and was in accordance with the company's instructions for use (IFU). 10,11 The anonymised CTA data set was segmented using OsirixLite (Pixmeo SARL, Switzerland). A reconstruction of the vasculature was imported into the CASE-IT software package (Mentice, Gothenburg, Sweden) and aligned to the bony landmarks.…”

“…12 The relevant vessel measurements had previously been estimated by six experienced EVAR operators. 10 The participants had access to the mean values of these estimates and the stent graft devices were chosen from the Bolton Treovance line at their own discretion. The participants were allowed to change any sub-parts during the procedure upon request.…”

“…Within the USA and most of Sweden the threshold is >25mm. 1 An aorta measuring 25-29mm has been defined as 'subaneurysmal'. The optimal screening strategy for these patients is unknown.…”
